目的：探讨邻近皮瓣联合游离植皮及负压封闭引流（VSD）修复慢性创面的临床应用价值。方法应用邻近皮瓣联合游离植皮及 VSD 修复慢性创面40例患者，其中男33例，女7例；年龄32～90岁，平均62．5岁，糖尿病足17例，压疮23例。创面大小5 cm ×8 cm ～15 cm ×20 cm，18例合并慢性骨髓炎，25例合并截瘫。创面床肉芽组织生长较新鲜后，采用邻近皮瓣修复裸露的创面，部分配合游离植皮，VSD 覆盖皮瓣及植皮区。术后对皮瓣移植区及游离植皮区的存活情况和愈合质量进行随访。结果40例慢性创面患者，应用邻近皮瓣联合游离植皮及 VSD 技术创面有效修复。术后随访6～24个月，平均12．8个月，皮瓣移植区及游离植皮区组织均成活且质地良好、外观满意。结论好对于慢性创面采用邻近皮瓣移植联合游离植皮及VSD 技术，可有效修复创面，明显缩短疗程，最大限度提高患者生活质量。
目的：探討鄰近皮瓣聯閤遊離植皮及負壓封閉引流（VSD）脩複慢性創麵的臨床應用價值。方法應用鄰近皮瓣聯閤遊離植皮及 VSD 脩複慢性創麵40例患者，其中男33例，女7例；年齡32～90歲，平均62．5歲，糖尿病足17例，壓瘡23例。創麵大小5 cm ×8 cm ～15 cm ×20 cm，18例閤併慢性骨髓炎，25例閤併截癱。創麵床肉芽組織生長較新鮮後，採用鄰近皮瓣脩複裸露的創麵，部分配閤遊離植皮，VSD 覆蓋皮瓣及植皮區。術後對皮瓣移植區及遊離植皮區的存活情況和愈閤質量進行隨訪。結果40例慢性創麵患者，應用鄰近皮瓣聯閤遊離植皮及 VSD 技術創麵有效脩複。術後隨訪6～24箇月，平均12．8箇月，皮瓣移植區及遊離植皮區組織均成活且質地良好、外觀滿意。結論好對于慢性創麵採用鄰近皮瓣移植聯閤遊離植皮及VSD 技術，可有效脩複創麵，明顯縮短療程，最大限度提高患者生活質量。
목적：탐토린근피판연합유리식피급부압봉폐인류（VSD）수복만성창면적림상응용개치。방법응용린근피판연합유리식피급 VSD 수복만성창면40례환자，기중남33례，녀7례；년령32～90세，평균62．5세，당뇨병족17례，압창23례。창면대소5 cm ×8 cm ～15 cm ×20 cm，18례합병만성골수염，25례합병절탄。창면상육아조직생장교신선후，채용린근피판수복라로적창면，부분배합유리식피，VSD 복개피판급식피구。술후대피판이식구급유리식피구적존활정황화유합질량진행수방。결과40례만성창면환자，응용린근피판연합유리식피급 VSD 기술창면유효수복。술후수방6～24개월，평균12．8개월，피판이식구급유리식피구조직균성활차질지량호、외관만의。결론호대우만성창면채용린근피판이식연합유리식피급VSD 기술，가유효수복창면，명현축단료정，최대한도제고환자생활질량。
Objective To explore the clinical value of combination of adjacent flap and free skin covered by vacuum sealing drainage (VSD)in the treatment of chronic wounds.Methods 40 cases with chronic wounds were repaired by combination of adjacent flap and free skin covered by VSD.There were 33 males and 7 females,with an average age of 62.5 years (ranged from 32 to 90 years),and 17 cases with diabetic foot and 23 cases with pressure sore.The size of wound varied from 5cm ×8cm -15cm ×20cm.Meantime,there were 18 cases with chronic osteomye-litis,and 25 cases with paraplegia.After wound bed grew freshly,wounds were repaired by combination of adjacent flap and free skin covered by VSD.The survival rate and the heal quality of the adjacent flap and the skin graft area were followed up postoperatively.Results 40 patients with chronic wounds were effectively repaired by combination of adjacent flap and free skin covered by VSD.The patients were followed up for an average of 12.8 months (6 -24months),the survival rate and the heal quality of the adjacent flap and the skin graft area were well and with satis-factory outline.Conclusion Patients with chronic wounds can be effectively treated with a combination of adjacent flap and free skin covered by VSD,which can effectively repaired of wound and significantly shortened the treatment, and maximized enhancement of the quality of life.
