目的:通过探讨氢质子磁共振波谱成像与足月新生儿缺氧缺血性脑病代谢变化的关系,以评估其在诊断新生儿缺氧缺血性脑病的应用价值。方法收集我院4年间经临床确诊为新生儿缺氧缺血性脑病的足月新生儿共37例为实验组,随机抽出同期在我院产科出生健康的新生儿共11人,为对照组。所有研究对象于出生11天内进行头颅氢质子磁共振波谱检查,测量右侧基底节各代谢产物波峰下面积值。在出生后6月时分别做发育商试,并对上述资料进行统计学分析。结果①新生儿缺血缺氧性脑病早期可引起颅内右侧基底节naa/cr、lac/cr、Glx/cr化合物的改变,且病变越重,相关的改变越明显。②新生儿缺血缺氧性脑病Mrs各代谢物的改变与预后高度相关。结论氢质子磁共振波谱对新生儿缺氧缺血性脑病的临床诊断具有重要临床意义,而且无创伤及辐射值得深入研究及推广。

Objective: to discuss relationship between proton magnetic resonance imaging and metabolism changes of mature neonates with hypoxic ischemic encephalopathy, and evaluate its application value of diagnosing neonatal hypoxic ischemic encephalopathy. Method: collect a total of 37 cases mature neonatal with definite clinical diagnosis of hypoxic ischemic encephalopathy in our hospital during the past 4 years as experiment group, and choose a total of 11 cases healthy neonatal at obstetrics department of our hospital randomly at the same period as control group. all cases were carried on head proton magnetic resonance spectroscopy within 11 days after birth, and test area under peak of right side basal ganglia metabolites. 6 months after birth, they were carried on development quotient, and analyze concern data statistically. Result:①neonatal hypoxic ischemic encephalopathy at early stage may cause compound changes of naa/cr, lac/cr and Glx/cr at right basal ganglia of brain, and changes are obvious with severe lesion.②changes of Mrs metabolites in neonatal hypoxic ischemic encephalopathy are highly correlated with prognosis. Conclusion:Proton magnetic resonance spectroscopy has important clinical signiifcance in diagnosing neonatal hypoxic ischemic encephalopathy, without injury and radiation, which is worthy of further study and promotion.
