目的:探讨颈总动脉粥样硬化疾病患者颈总动脉硬化程度与左心功能的相关性。方法应用QIMT和QAS技术对CAS患者75例与健康人群50例自动测量颈总动脉的内中膜厚度(IMT)、血管扩张性系数(DC)、血管顺应性系数(CC)、僵硬度参数(β)和脉搏波传导速度(PWV)。应用超声心动图测量室间隔舒张末期厚度(IVSd)、左室舒张末期内径(LVDd)、左室质量(LVM)、射血分数(EF)、 E/A、 E/E'。对比所测量结果并进行相关分析。结果 CAS患者颈总动脉IMT、β、 PWV显著高于健康人群, DC、 CC显著低于健康人群; CAS患者IVSd、 LVM、 E/E'显著高于健康人群;差异均具有统计学意义(P <0.05)。 CAS患者IMT、 PWV及β与CC呈负相关,与E/E'呈正相关。结论 CAS患者动脉僵硬度增高,同时伴有左室舒张功能受损,二者之间呈正相关性。

목적:탐토경총동맥죽양경화질병환자경총동맥경화정도여좌심공능적상관성。방법응용QIMT화QAS기술대CAS환자75례여건강인군50례자동측량경총동맥적내중막후도(IMT)、혈관확장성계수(DC)、혈관순응성계수(CC)、강경도삼수(β)화맥박파전도속도(PWV)。응용초성심동도측량실간격서장말기후도(IVSd)、좌실서장말기내경(LVDd)、좌실질량(LVM)、사혈분수(EF)、 E/A、 E/E'。대비소측량결과병진행상관분석。결과 CAS환자경총동맥IMT、β、 PWV현저고우건강인군, DC、 CC현저저우건강인군; CAS환자IVSd、 LVM、 E/E'현저고우건강인군;차이균구유통계학의의(P <0.05)。 CAS환자IMT、 PWV급β여CC정부상관,여E/E'정정상관。결론 CAS환자동맥강경도증고,동시반유좌실서장공능수손,이자지간정정상관성。
Objective To evaluate the correlation between the degree of arteriosclerosis and left ventricular function in patients with carotid atherosclerosis (CAS). Methods The intima-media thickness (IMT), vessel distensibility coefficient (DC), vessel compliance coefficient (CC), stiffnessβand pulse wave velocity (PWV) of 75 CAS patients and 50 healthy subjects were measured by QIMT and QAS. The IVSd, LVDd, LVM, EF, E/A and E/E' were measured by echocardiography. The measuring results of two groups were compared and analyzed. Results The IMT,β, PWV, IVSd, LVM and E/E' of CAS patients were significantly higher than those of healthy subjects, the DC and CC were significantly lower than those of healthy subjects, with statistical difference (P<0.05). The IMT, PWV andβof CAS patients had negative correlation with CC, and had positive correlation with E/E'. Conclusions CAS patients have high artery stiffness, and impaired left ventricular function. There results show positive correlation between CAS and left ventricular dysfunction.
