目的：研究右美托咪定在儿童全凭静脉麻醉苏醒期躁动中的预防作用。方法将100例择期行扁桃体腺样体切除术的患儿随机分为两组各50例，实验组麻醉诱导后持续静脉泵入右美托咪定(0.2μg?kg-1?h-1)，对照组持续泵入等量生理盐水。记录手术时间、麻醉时间、停药至睁眼时间、恢复室停留时间，比较两组PAED评分、 FLACC评分、脸谱疼痛评分以及不良事件发生情况。结果两组手术时间、麻醉时间、睁眼时间、恢复室停留时间、 FLACC评分以及不良反应发生率比较差异不显著(P＞0.05)；实验组PAED评分和脸谱疼痛评分均显著低于对照组，差异具有统计学意义(P＜0.05)。结论右美托咪定可在一定程度上预防儿童全凭静脉麻醉苏醒期的躁动，且不延长麻醉苏醒时间和增加不良反应。
목적：연구우미탁미정재인동전빙정맥마취소성기조동중적예방작용。방법장100례택기행편도체선양체절제술적환인수궤분위량조각50례，실험조마취유도후지속정맥빙입우미탁미정(0.2μg?kg-1?h-1)，대조조지속빙입등량생리염수。기록수술시간、마취시간、정약지정안시간、회복실정류시간，비교량조PAED평분、 FLACC평분、검보동통평분이급불량사건발생정황。결과량조수술시간、마취시간、정안시간、회복실정류시간、 FLACC평분이급불량반응발생솔비교차이불현저(P＞0.05)；실험조PAED평분화검보동통평분균현저저우대조조，차이구유통계학의의(P＜0.05)。결론우미탁미정가재일정정도상예방인동전빙정맥마취소성기적조동，차불연장마취소성시간화증가불량반응。
Objective To study the preventive effect of dexmedetomidine on agitation of children with total intravenous anesthesia in recovery period. Methods 100 children with tonsil adenoidectomy were randomly divided into two groups, with 50 cases in each group. The experimental group was continuously-pumped dexmedetomidine (0.2μg?kg-1?h-1), the control group was continuously-pumped equivalent normal saline. The operation time, anesthesia time, eye-opening time and length of stay in recovery room were recorded, the PAED score, FLACC score and Facebook pain score and incidence of adverse events were compared between two groups. Results The operation time, anesthesia time, eye-opening time, length of stay in recovery room, FLACC score and incidence of adverse events in two groups had no statistical difference (P >0.05). The PAED score and Facebook pain score of experimental group were significantly lower than those in control group, with statistical difference (P <0.05). Conclusions Dexmedetomidine can prevent the agitation of children with total intravenous anesthesia in recovery period, without prolonging anesthesia waking time and increasing adverse reactions.
