目的探讨血清淀粉酶、脂肪酶及C反应蛋白联合检验在急性胰腺炎诊断中的应用价值。方法选择轻型胰腺炎患者、重型胰腺炎患者、健康体检者作为轻型胰腺炎组、重型胰腺炎组以及对照组,检测血清中淀粉酶、脂肪酶及C反应蛋白含量,观察临床症状和体征的缓解情况。结果轻型胰腺炎组和重型胰腺炎组患者的血清淀粉酶、脂肪酶以及C反应蛋白的含量均高于对照组,重型胰腺炎组明显高于轻型胰腺炎组(t=15.706~51.077,P<0.05);三项指标联合检测的灵敏性、特异性、准确度均高于一项指标检测或两项指标检测(χ2=4.325~8.547,P<0.05);重型胰腺炎组腹痛、发热、恶心、呕吐、腹膜刺激征缓解时间明显长于轻型胰腺炎组(t=7.719~15.384,P<0.05);血清C反应蛋白含量与腹痛、发热、恶心、呕吐、腹膜刺激征缓解时间呈正相关(β=0.894~1.482,R=0.695~0.803,t=11.541~18.582,P<0.05)。结论血清淀粉酶、脂肪酶及C反应蛋白联合检测能够提高急性胰腺炎诊断的灵敏性、特异性、准确度,且C反应蛋白含量能够准确判断病情。

목적탐토혈청정분매、지방매급C반응단백연합검험재급성이선염진단중적응용개치。방법선택경형이선염환자、중형이선염환자、건강체검자작위경형이선염조、중형이선염조이급대조조,검측혈청중정분매、지방매급C반응단백함량,관찰림상증상화체정적완해정황。결과경형이선염조화중형이선염조환자적혈청정분매、지방매이급C반응단백적함량균고우대조조,중형이선염조명현고우경형이선염조(t=15.706~51.077,P<0.05);삼항지표연합검측적령민성、특이성、준학도균고우일항지표검측혹량항지표검측(χ2=4.325~8.547,P<0.05);중형이선염조복통、발열、악심、구토、복막자격정완해시간명현장우경형이선염조(t=7.719~15.384,P<0.05);혈청C반응단백함량여복통、발열、악심、구토、복막자격정완해시간정정상관(β=0.894~1.482,R=0.695~0.803,t=11.541~18.582,P<0.05)。결론혈청정분매、지방매급C반응단백연합검측능구제고급성이선염진단적령민성、특이성、준학도,차C반응단백함량능구준학판단병정。
Objective To study the application value of serum amylase, lipase, C-reactive protein combined detec-tion in the diagnosis of acute pancreatitis. Method Light pancreatitis, severe pancreatitis patients and healthy people were enrolled in light pancreatitis group, severe pancreatitis group and control group. Then serum amylase, lipase and C-reactive protein were detected, clinical symptoms and signs remission were observed. Result Light pancreati-tis group and severe pancreatitis group serum amylase, lipase, C-reactive protein levels were significantly higher than those of control group, severe pancreatitis group were significantly higher than light pancreatitis group ( t=15. 706~51. 077,P<0. 05);three indicators combined detection sensitivity, specificity, accuracy were significantly higher than one index detection or two indicators detection(χ2=4. 325 ~8. 547, P<0. 05); severe pancreatitis group abdominal pain, fever, nausea, vomiting, peritoneal irritation remission time were significantly longer than light pancreatitis group(t=7. 719~15. 384,P<0. 05), serum C-reactive protein was positively correlated with ab-dominal pain, fever, nausea, vomiting, peritoneal irritation remission time(β=0. 894~1. 482,R=0. 695~0. 803, t=11. 541~18. 582,P<0. 05). Conclusion Serum amylase, lipase and C-reactive protein combined detection can improve sensitivity, specificity, accuracy of acute pancreatitis diagnosis;C-reactive protein can accurately determine the disease condition.
