目的：探讨降钙素原（PCT）、白细胞介素－6（IL－6）、白细胞介素－8（IL－8）等对危重症患者呼吸机相关性肺炎（ VAP）早期诊断的价值。方法选择2013年1月2014年12月行有创机械通气的危重症患者120例，其中发生VAP的60例为VAP组，未发生VAP的60例为N－VAP组，比较两组患者PCT、IL－6、IL－8、C－反应蛋白（ CRP）、白细胞计数（ WBC）检测值。结果 VAP组PCT、IL－6、IL－8、CRP、WBC、体温与N－VAP组比较差异有统计学意义（P＜0．05），同时比较VAP组患者中康复组和恶化组，两组患者的PCT、IL－6、IL－8、WBC、CRP比较差异有统计学意义（ P＜0．05）。结论 PCT、IL－6和IL－8对危重症患者VAP早期诊断具有重要的临床价值。
목적：탐토강개소원（PCT）、백세포개소－6（IL－6）、백세포개소－8（IL－8）등대위중증환자호흡궤상관성폐염（ VAP）조기진단적개치。방법선택2013년1월2014년12월행유창궤계통기적위중증환자120례，기중발생VAP적60례위VAP조，미발생VAP적60례위N－VAP조，비교량조환자PCT、IL－6、IL－8、C－반응단백（ CRP）、백세포계수（ WBC）검측치。결과 VAP조PCT、IL－6、IL－8、CRP、WBC、체온여N－VAP조비교차이유통계학의의（P＜0．05），동시비교VAP조환자중강복조화악화조，량조환자적PCT、IL－6、IL－8、WBC、CRP비교차이유통계학의의（ P＜0．05）。결론 PCT、IL－6화IL－8대위중증환자VAP조기진단구유중요적림상개치。
Objective To investgate the early diagnosis value of procalcitonin(PCT), interleukin -6(IL-6), in-terleukin -8(IL-8) on ventilator-associated pneumonia of critically ill patients .Methods One hundred and twenty patients with invasive mechanical ventilation were divided into two groups( the VAP group and the N-VAP group) , with 60 cases in each group.PCT, IL-6, IL-8, C reactive protein(CRP), white blood cell count((WBC), temperature(T) were detected in the two groups.Results There were significant differences in PCT, IL-6, IL-8,CRP, WBC and T between the two groups (P<0.05).Comparison of VAP between the rehabilitation group and the deterioration group, PCT, IL-6, IL-8, WBC, T and CRP of the two groups also had significant differences(P<0.05).Conclusions PCT,IL-6 and IL-8 have important clinical diagnostic value for the early diagnosis VAP of critically ill patients.
