目的探讨利福平耐药实时荧光定量核酸扩增检测技术( Xpert MTB/RIF,简称Xpert)在骨关节结核诊断中的临床应用价值。方法选取139例可疑诊断骨关节结核患者的脓液,以改良罗氏培养或病理结果为金标准,评估比较抗酸染色、结核分枝杆菌BACTEC MGIT 960培养(简称MGIT 960)、改良罗氏培养(简称罗培)和Xpert检测结核分枝杆菌的效能。结果抗酸染色法、MGIT 960法、罗培法和xpert在骨关节结核组的阳性率分别为20.18%(22/109),37.50%(36/96),25.23(28/111),76.93%(100/130)。 Xpert的阳性率均高于抗酸染色法、MGIT 960法及罗培法。结论Xpert MTB/RIF可用于骨关节结核的快速检测具有良好的应用前景。

Objective To investigate clinical value of Xpert Mycobacterium tuberculosis/Rifampicin ( Xpert Mtb/RIF) assay on detection of bone and joint tuberculous( BJTB) . Method 139 pus samples were obtained from suspec-ted BJTB patients. These samples were detected by smear, BACTEC MGIT 960 Mycobacterial Detection ( MGIT 960),Lowenstein-Jensen(L-J)culture (solid culture) and Xpert assay. The positive rates of Xpert assay in detec-ting BJTB were analyzed. Result According to the solid culture result or pathological result as gold standard, The positive rates of smear,MGIT 960,solid culture and Xpert were 20. 18%(22/109), 37. 50%(36/96), 25. 23(28/111), 76. 93%(100/130) in the BJTB group. The positive rate of abscess detected by Xpert was 76. 93%(100/130),it is higher than the other tests respectively. Conclusion Xpert assay has a good application prospect in rapid-ly detecting BJTB.
