目的：研究急性心肌梗死心血管内科的诊断和临床治疗的特点和效果。方法选取襄垣县人民医院2013年4月至2014年4月收治的40例急性心肌梗死患者作为研究对象，随机将患者分为两组，普通治疗组（对照组）和瑞替普酶组（试验组）。对照组20例患者应用常规药物诊治，试验组20例患者应用注射瑞替普酶治疗，治疗疗程为4个月，记录并观察两组患者的各项生理指标的变化，4个月后，将两组患者的临床疗效进行对比，并且根据治疗结果，将两组患者治疗前后的各项指标进行对比。结果经过4个月的治疗观察，对照组和试验组的治疗有效率分别为60．00％和85．00％，差异有统计学意义（ P＜0．05）。两组患者治疗后左室舒张期内径对照组为（67．25±7．23）mm，试验组为（68．01±5．12）mm；左室收缩期内径对照组为（58．12±6．23）mm，试验组为（59．21±7．23） mm；左室后壁厚度对照组为（9．01±1．23） mm，试验组为（8．91±2．21）mm；C－反应蛋白对照组为（15．85±3．22）mg／L，试验组为（16．02±2．32）mg／L。两组数据相比较，差异有统计学意义（P＜0．05）。两组患者出现不良反应的病例对照组为12例，试验组为5例，差异有统计学意义（ P＜0．05）。结论对急性心肌梗死患者注射瑞替普酶治疗有很好的临床效果，较常规治疗方法有极高的疗效，有极佳的治疗作用。
목적：연구급성심기경사심혈관내과적진단화림상치료적특점화효과。방법선취양원현인민의원2013년4월지2014년4월수치적40례급성심기경사환자작위연구대상，수궤장환자분위량조，보통치료조（대조조）화서체보매조（시험조）。대조조20례환자응용상규약물진치，시험조20례환자응용주사서체보매치료，치료료정위4개월，기록병관찰량조환자적각항생리지표적변화，4개월후，장량조환자적림상료효진행대비，병차근거치료결과，장량조환자치료전후적각항지표진행대비。결과경과4개월적치료관찰，대조조화시험조적치료유효솔분별위60．00％화85．00％，차이유통계학의의（ P＜0．05）。량조환자치료후좌실서장기내경대조조위（67．25±7．23）mm，시험조위（68．01±5．12）mm；좌실수축기내경대조조위（58．12±6．23）mm，시험조위（59．21±7．23） mm；좌실후벽후도대조조위（9．01±1．23） mm，시험조위（8．91±2．21）mm；C－반응단백대조조위（15．85±3．22）mg／L，시험조위（16．02±2．32）mg／L。량조수거상비교，차이유통계학의의（P＜0．05）。량조환자출현불량반응적병례대조조위12례，시험조위5례，차이유통계학의의（ P＜0．05）。결론대급성심기경사환자주사서체보매치료유흔호적림상효과，교상규치료방법유겁고적료효，유겁가적치료작용。
Objective To investigate the characteristics and effects of clinical diagnosis and treatment of acute myocardial infarction in the cardiovascular department.Methods From April 2013 to April 2014, forty patients with acute myocardial infarction were selected.The patients were randomly divided into general treatment group ( control group) and the reteplase group( test group) .The 20 patients in control group were given conventional drug enforce-ment treatment, and patients in experiment group were injected with reteplase therapy, the treatment course was 4 months.The changes of the physiological indexes of the two groups of patients were recorded, 4 months later, the clin-ical effects of the two groups were compared, and according to the results of treatment, each index of two groups of pa-tients before and after treatment was compared.Results After 4 months of treatment, the effective rate was 60% in control group and 85% in test group, the difference was significant ( P<0.05) .The left ventricular diastolic diame-ter of control group was(67.25 ±7.23)mm, that of the test group was(68.01 ±5.12)mm; the left ventricular sys-tolic diameter of control group was(58.12 ±6.23)mm, that of the test group was(59.21 ±7.23)mm; the left ven-tricular posterior wall thickness of control group was(9.01 ±1.23)mm, that of the test group was(8.91 ±2.21)mm;the level of C-reactive protein of control group was (15.85 ±3.22) mg/L, that of the test group was(16.02 ±2.32) mg/L, and there were significant differences ( P<0.05) .Twelve patients had adverse reactions in control group, and 5 patients had adverse reactions in test group, there was significant difference(P<0.05).Conclusions The clinical effects of in-jection of reteplase on patients with acute myocardial infarction is good, it has better effect than conventional treatment, has excellent treatment function.
