目的:观察长航期间短期靠港对舰员24 h动态心电图变化的影响。方法对30名短期靠港舰员行24 h动态心电图的窦性心律、期前收缩、ST段、QT间期离散度和心率变异性的时域、频域、非线性指标变化的分析。结果与靠港前比较,靠港后的平均心率减慢、最高心率加快、最低心率减慢、最长R-R间期次数减少、最长R-R间期时间缩短,期前收缩减少、MV5导联ST段压低减轻及压低持续时间缩短,全部窦性心搏R-R间期标准差、每5 min时段R-R间期平均值的标准差、每5 min时段R-R间期平均值的标准差指数、相邻正常R-R间期差值的均方根、相邻正常R-R间期差值>50 ms的心搏数、相邻正常R-R间期差值>50 ms的百分比、三角指数、甚低频、甚高频、矢量角度指数及矢量长度指数明显增加。结论脱离作业环境可促进支配心脏的自主神经功能恢复。

Objective To observe the effects of short periods of making berth on 24 h holter changes of the crew during oceangoing voyage.Methods For 30 cases of short-term port crew, sinus rhythm, premature beat, ST segment, QT dispersion and time domain,frequency domain and nonlinear index changes of heart rate variability were analyzed.Results Compared with out operating environ-ment, the average heart rate and the lowest heart beat decreased, the highest heart beat increased, the sinus node dysfunction improved, premature contraction were less frequent, the depressed ST segment elevated, the standard deviation of normal-to-normal intervals ( SDNN) , standard deviation of the average NN in all 5 min of the entire recording ( SDANN) , SDNN index, root mean square of successive differences (RMSSD), number of pairs of adjacent normal-to-normal intervals by more than50 ms (NN50), percent of NN50 (PNN50), triangular index, very low frequency (VLF), very high frequency ( VHF) , vector angle index ( VAI) and vector length index ( VLI) increased. Conclusion Changing of operating environment can promote the recovery of autonomic nervous system.
