目的:观察应用白虎汤后全身炎症反应综合征( SIRS )大鼠血浆TNF-α水平及肺组织的变化,探讨白虎汤对发生SIRS后肺组织的影响及机制。方法将60只大鼠随机分为对照组、模型组、中药组及西药组,每组各15只。模型组、中药组及西药组均行腹腔注射大肠杆菌脂多糖20 mg/kg制备SIRS模型。对照组不造模。模型组、西药组、中药组于造模后分别予生理盐水灌胃、氢化可的松琥珀酸钠静脉注射及白虎汤灌胃,共3d。造模后第4日,各组大鼠腹主动脉采血,采用ELISA法检测血浆TNF-α水平。随后处死大鼠取肺组织,肉眼及HE染色后光镜下观察肺组织病理表现。结果对照组、模型组、西药组、中药组血浆TNF-α水平分别为(64.05±7.72)、(74.25±8.67)、(62.63±7.01)、(58.78±9.68)pg/mL,模型组TNF-α水平均高于对照组、中药组及西药组(P均<0.05)。光镜下,对照组肺组织结构完整,肺泡腔清晰,肺泡隔无水肿、炎细胞浸润等改变;模型组肺间质充血,伴炎细胞浸润,肺泡隔明显增厚;中药组、西药组肺组织病理改变均较模型组减轻,且中药组改善程度更好。结论白虎汤对SIRS大鼠肺组织具有保护作用,其机制可能与减少TNF-α的释放有关。

목적:관찰응용백호탕후전신염증반응종합정( SIRS )대서혈장TNF-α수평급폐조직적변화,탐토백호탕대발생SIRS후폐조직적영향급궤제。방법장60지대서수궤분위대조조、모형조、중약조급서약조,매조각15지。모형조、중약조급서약조균행복강주사대장간균지다당20 mg/kg제비SIRS모형。대조조불조모。모형조、서약조、중약조우조모후분별여생리염수관위、경화가적송호박산납정맥주사급백호탕관위,공3d。조모후제4일,각조대서복주동맥채혈,채용ELISA법검측혈장TNF-α수평。수후처사대서취폐조직,육안급HE염색후광경하관찰폐조직병리표현。결과대조조、모형조、서약조、중약조혈장TNF-α수평분별위(64.05±7.72)、(74.25±8.67)、(62.63±7.01)、(58.78±9.68)pg/mL,모형조TNF-α수평균고우대조조、중약조급서약조(P균<0.05)。광경하,대조조폐조직결구완정,폐포강청석,폐포격무수종、염세포침윤등개변;모형조폐간질충혈,반염세포침윤,폐포격명현증후;중약조、서약조폐조직병리개변균교모형조감경,차중약조개선정도경호。결론백호탕대SIRS대서폐조직구유보호작용,기궤제가능여감소TNF-α적석방유관。
Objective To observe the effect of Baihu decoction on the changes of TNF-ɑand lung tissues of rats with systemic inflammatory response syndrome ( SIRS) , and to investigate the influence and mechanism of Baihu decoction on the lung tissues of the SIRS model rats .Methods Sixty rats were randomly divided into the control group , model group , and Chinese medicine Baihu decoction treatment group ( Chinese medicine group ) and Western medicine hydrocortisone so-dium succinate treatment group ( Western medicine group ) , each group had fifteen rats .The SIRS model rats were made in the model group, Chinese medicine and Western medicine group by escherichia coli lipopolysaccharide (20 mg/kg).After modeling , the rats of the model group , Western medicine group and the Chinese medicine group were respectively treated with normal saline by gavage, hydrocortisone sodium succinate and Baihu decoction by vein for 3 days.On the fourth day, the rat abdominal aortic blood of each group was acquired , and the plasma TNF-ɑlevel was measured by enzyme-linked im-munosorbent assay (ELISA), then we killed the rats and took out the lung tissues .The pathological changes of the lung tissues were observed by the naked eyes and light microscopy after HE staining .Results The levels of TNF-ɑof the con-trol group, model group, and Chinese medicine group and Western medicine group were respectively (64.05 ±7.72), (74.25 ±8.67), (62.63 ±7.01) and (58.78 ±9.68) pg/mL;the TNF-ɑlevel of the model group was higher than that of the control group (P<0.05), the TNF-ɑlevels of the Chinese medicine group and Western medicine group were lower than that of the model group (P<0.05).Under the light microscopy, the lung tissues of the control group had a structural integrity, clear alveolar space and alveolar septa without edema , no inflammatory cell infiltration and other changes; the model group had interstitial pulmonary congestion complicated with inflammatory cell infiltration , and thickened alveolar septa.The histopathological changes of the Chinese medicine and Western medicine group were significantly lighter than that of the control group , and the histopathological change of the Chinese medicine group was better than that of the Western medicine group .Conclusion Baihu decoction has protective effect on the lung tissues of SIRS rats , and the mechanism may be related to decrease of TNF-αrelease.
