目的：：分析慢性鼻-鼻窦炎( CRS)患者外周血补体C3、C4的变化及其与CRS的关系。方法：回顾性分析2012年3月至2014年5月本科收治的188例CRS手术患者的临床资料,其中不伴鼻息肉组( CRSsNP )97例、伴鼻息肉组( CRSwNP )91例；选取同期本院体检中心的58例健康体检者(无既往疾病史)作为对照组。检测外周血补体C3、C4水平,并与CRS患者的临床资料进行相关分析。结果：28例(28.87％)CRSsNP患者、35例(38.46％)CRSwNP患者外周血C3水平下降,两组比较,差异无统计学意义(P>0.05),但均明显高于正常对照组(3例,5.17％,P<0.05)。37例(38.14％)CRSsNP、49例 CRSwNP (53.84％)患者外周血C4水平下降,两组比较,差异有统计学意义(P<0.05),且均高于正常对照组(2例,3.45％,P<0.05)。 CRSsNP和CRSwNP患者外周血C3和C4水平均成正相关(r＝0.412,0.418,P<0.05)；CRSwNP患者外周血C3与中性粒细胞数及CT评分具有明显的相关性( r＝0.260,P<0.05)。结论：补体系统可能在CRS的发病机制中发挥重要作用,不同亚型的CRS中补体系统的活化途径存在差异。

목적：：분석만성비-비두염( CRS)환자외주혈보체C3、C4적변화급기여CRS적관계。방법：회고성분석2012년3월지2014년5월본과수치적188례CRS수술환자적림상자료,기중불반비식육조( CRSsNP )97례、반비식육조( CRSwNP )91례；선취동기본원체검중심적58례건강체검자(무기왕질병사)작위대조조。검측외주혈보체C3、C4수평,병여CRS환자적림상자료진행상관분석。결과：28례(28.87％)CRSsNP환자、35례(38.46％)CRSwNP환자외주혈C3수평하강,량조비교,차이무통계학의의(P>0.05),단균명현고우정상대조조(3례,5.17％,P<0.05)。37례(38.14％)CRSsNP、49례 CRSwNP (53.84％)환자외주혈C4수평하강,량조비교,차이유통계학의의(P<0.05),차균고우정상대조조(2례,3.45％,P<0.05)。 CRSsNP화CRSwNP환자외주혈C3화C4수평균성정상관(r＝0.412,0.418,P<0.05)；CRSwNP환자외주혈C3여중성립세포수급CT평분구유명현적상관성( r＝0.260,P<0.05)。결론：보체계통가능재CRS적발병궤제중발휘중요작용,불동아형적CRS중보체계통적활화도경존재차이。
Objective:To investigate the changes of complement C3 and C4 in peripheral blood ( PB) of patients with chronic rhinosinusitis ( CRS) and its correlation with CRS. Methods:The clinical data of 188 CRS patients hospitalized in our deparment between March 2012 and May 2014 were retrospectively analyzed, and divided into the CRS without nasal polyps (CRSsNP) group (n=97) and the CRS with nasal polyps (CRSwNP) group (n=91). A contemporary cohort of 58 healthy subjects (no previous medical history) examined in our physical examination center was included in the control group. The complement C3 and C4 levels in PB were determined for correlation analysis with the clinical data of the CRS patients. Results:The C3 level in PB of 28 CRSsNP patients ( 28. 87%) and 35 CRSwNP patients ( 38. 46%) were decreased without statistically significant differences (P>0.05), and were significantly higher than that in the control normal group (3 cases, 5.17%, P<0. 05). The C4 level in PB of 37 CRSsNP patients (38.14%) and 49 CRSwNP patients (53.84%) were decreased with statistically significant differences (P<0.05), and were significantly higher than that in the control normal group (2 cases, 3.45%, P<0.05). The C3 and C4 levels in PB of CRSsNP and CRSwNP were positively correlated (r=0.412,0.418,P<0.05). Significant correlation was found between C3 level in PB, neutrophil and CT score in CRSwNP patients (r=0.60,P<0.05). Conclusion:The complement system may play an important role in the pathogenesis of CRS. There are activation pathways in the complement system of different subtypes of CRS.
