目的 联合应用功能磁共振成像( fMRI)和DTI探讨右侧颞叶癫痫患者( TLE)执行功能与脑结构损害的关系. 方法 采用注意网络测试对11例右侧TLE患者(右侧TLE组)及9名健康受试者(健康组)进行行为学练习与测试,并进行执行功能的fMRI及DTI扫描. 采用统计学参数图分析fMRI的数据,采用DTI Studio软件分析DTI的数据. 结果 右侧TLE组执行功能时右侧前额叶及右侧顶叶可见激活,激活脑区小于健康组. 右侧TLE组右侧前额叶激活强度较健康组减弱( P<0.001 ). 与健康组比较,右侧TLE组右侧扣带束明显短小,扣带束各向异性值( FA值)、长度值( L值)明显降低(均P<0.05 ). 右侧TLE组右侧前额叶激活像素数目与右侧扣带束FA值(r=0.715,P=0.013)及L值(r=0.677,P=0.022)呈正相关. 结论右侧TLE患者执行功能网络受损,其右侧前额叶的激活强度与右侧扣带束纤维和结构的完整性有关.

Objective To explore the relationship between executive function and struction damage in patients with right temporal lobe epilepsy ( TLE ) by functional magnetic resonance imaging ( fMRI ) combined with DTI. Methods Attention network test was used to practice and test the behavior of of 11 patients with right TLE ( right TLE group) and 9 healthy controls ( healthy group ) , and then fMRI and DTI were used to scan the executive function.The fMRI data was analyzed by statistical parameters of the figure, and the DTI data was analyzed by the DTI studio software.Results Right prefrontal lobe and right parietal lobe were activated by executive function in right TLE group, and the areas of activated brain were smaller than healthy group.The intensity of activation of right prefrontal lobe in right TLE group were significantly weaker than that in healthy group (P<0.001).Compared with healthy group, the right cingulum was shorter and smaller in right TLE group, and the fractional anisotropy value ( FA value) and length value (L value) were significantly decreased (all P<0.05).The pixel number of activation of right prefrontal lobe in right TLE group was positively correlated with the FA value ( r=0.715,P=0.013) and the L value (r=0.677,P=0.022) of right cingulum.Conclusions The executive network in right TLE patient is damaged.The intensity of activation of right prefrontal lobe in which is correlated with the integrity of the right cingulum fiber and structure.
