目的:了解维吾尔族和汉族老年人血清骨保护素( OPG)水平与骨量的关系。方法于2011年10月—2013年12月,选取在新疆医科大学第一附属医院干部病房接受住院治疗的维吾尔族和汉族老年人375例。根据骨密度(BMD)测定结果,将其分为骨量异常组(n=239)和骨量正常组(n=136)。比较不同骨量情况、民族、性别、年龄老年人的血清OPG水平,并探讨血清OPG水平与骨量的相关关系。结果(1)骨量正常组与骨量异常组老年人血清OPG水平比较,差异有统计学意义(P<0.05)。按民族分层:两组老年人血清OPG水平比较,差异无统计学意义(P>0.05)。按性别分层:两组男性老年人血清OPG水平比较,差异有统计学意义(P<0.05);而两组女性老年人血清OPG水平比较,差异无统计学意义(P>0.05)。(2)不同民族老年人血清OPG水平比较,差异有统计学意义(P<0.05)。按性别分层:不同民族男性老年人血清OPG水平比较,差异有统计学意义(P<0.05);不同民族女性老年人血清OPG水平比较,差异无统计学意义( P>0.05)。按骨量正常与否分层:不同民族骨量正常老年人血清OPG水平比较,差异有统计学意义( P<0.05);不同民族骨量异常老年人血清OPG水平比较,差异无统计学意义( P>0.05)。(3)不同性别老年人血清OPG水平比较,差异无统计学意义(P>0.05)。按民族和骨量正常与否分层后,不同性别老年人血清OPG水平比较,差异亦无统计学意义(P>0.05)。(4)不同年龄老年人血清OPG水平比较,差异无统计学意义(P>0.05)。按民族和骨量正常与否分层后,不同年龄老年人血清OPG水平比较,差异亦无统计学意义(P>0.05)。(5) Spearman秩相关分析显示:汉族男性老年人和维吾尔族女性老年人血清OPG水平与骨量无相关关系(P>0.05);维吾尔族男性老年人血清OPG水平与骨量存在正相关关系(P<0.05);而汉族女性老年人血清OPG水平与股骨颈、大转子、小转子及髋关节部位骨量存在负相关关系(P<0.05)。结论维吾尔族老年人的血清OPG水平低于汉族老年人;骨量异常老年人的血清OPG水平低于骨量正常老年人。维吾尔族男性老年人的血清OPG水平与骨量呈正相关关系,而汉族女性老年人的血清OPG水平与股骨颈、大转子、小转子及髋关节部位的骨量呈负相关关系。

Objective To explore the correlation between serum osteoprotegerin ( OPG ) and bone mineral density among Uygur and Han elderly populations. Methods We enrolled 375 Uygur and Han elderly people who were admitted into the cadre′s ward of the First Affiliated Hospital of Xinjiang Medical University from October 2011 to December 2013. According to the examination results of BMD, the subjects were divided into two groups: abnormal BMD group (n=239) and normal BMD group ( n=136 ) . Comparison was made in serum OPG level among subjects with different BMD conditions, ethnic groups, genders and ages, and the correlation between serum OPG level and BMD was investigated. Results ( 1 ) The two groups were significantly different (P<0. 05) in serum OPG level. The two groups were not significantly different (P>0. 05) in the serum OPG level of Uygur subjects or Han subjects. The two groups were significantly different ( P<0. 05 ) in the serum OPG level of male subjects, while no significant difference (P>0. 05) was noted in the serum OPG level of female subjects between the two groups. (2) There was significant difference (P<0. 05) in serum OPG level between Uygur subjects and Han subjects. Uygur subjects and Han subjects were significantly different (P<0. 05) in the serum OPG level of male subjects, while no significant difference ( P>0. 05 ) was found in the serum OPG level of female subjects between Uygur subjects and Han subjects. Uygur subjects and Han subjects were significantly different ( P <0. 05 ) in the serum OPG level of normal BMD group, while no significant difference ( P>0. 05 ) was noted in the serum OPG level of abnormal BMD group between Uygur subjects and Han subjects. (3) Subjects of two genders were not significantly different (P>0. 05) in serum OPG level. Between Uygur subjects and Han subjects and between normal BMD group and abnormal BMD group, there was no significant difference (P>0. 05) in the serum OPG level of each gender. (4) Subjects with different ages were not significantly different (P>0. 05) in serum OPG level. Between Uygur subjects and Han subjects and between normal BMD group and abnormal BMD group, no significant difference (P>0. 05) was noted in the serum OPG level of subjects of each age range. (5) Spearman rank correlation showed that no correlation (P>0. 05) was found between serum OPG level and BMD in Han male subjects and Uygur female subjects;there was positive correlation (P<0. 05) between serum OPG level and BMD in Uygur male subjects; negative correlation (P<0. 05) was noted between serum OPG level and BMD in neck of femur, greater trochanter, lesser trochanter and hip joint among Han female subjects. Conclusion Uygur elder people are lower than Han elder people in serum OPG level, and the elderly with abnormal BMD are lower than the elderly with normal BMD in serum OPG. There is positive correlation between serum OPG level and BMD in Uygur male elderly people, and there is negative correlation between serum OPG level and BMD in neck of femur, greater trochanter, lesser trochanter and hip joint among Han female elderly people.
