目的:探讨静脉丙种球蛋白联合甲泼尼龙治疗麻疹合并重症肺炎的临床疗效。方法整群选取该院2012年12月—2014年12月收治的麻疹合并重症肺炎患儿94例,按照编号随机分成观察组(n=47)和对照组(n=47),对照组采取静脉丙种球蛋白治疗,观察组则加用甲泼尼龙,比较两组临床疗效。结果观察组呼吸困难恢复时间、肺部啰音消失时间、血气恢复时间、胸片显示明显吸收时间、退热时间均短于对照组,差异有统计学意义(P<0.05)。观察组机械通气使用率0.00%,死亡率2.13%,对照组机械通气使用率10.64%,死亡率19.15%,观察组机械通气使用率及死亡率均低于对照组,差异有统计学意义(P<0.05)。观察组痊愈率97.87%,对照组痊愈率80.85%,观察组痊愈率高于对照组,差异有统计学意义(P<0.05)。观察组患儿在治疗过程中,不良反应发生率0.00%;对照组患儿在治疗过程中,不良反应发生率2.13%,两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论静脉丙种球蛋白联合甲泼尼龙治疗麻疹合并重症肺炎的临床效果较单独丙种球蛋白注射更好,且较为安全,临床价值较高。

Objective To observe the clinical curative effect of intravenous gamma globulin combined with methylprednisolone in the treatment of measles complicated with severe pneumonia. Methods Select our hospital supplementary information to select the starting and ending date 2012-12/2014-12 were measles combined with severe pneumonia in 94 cases, according to numbers were randomly divided into observation group (n=47) and control group (n=47), control group was treated with intravenous immunoglobulin therapy, observation group were treated with methylprednisolone, compared two groups of clinical curative effect. Results For the observation group, the time period for difficulty breathing recovery, pulmonary rales disappear, blood gas recovery, chest-X-ray-showed absorption, and cooling were shorter than those of the control group, with statistically significant differences(P<0.05). Mechanical ventilation rate and mortality rate of the observation group were 0.00% and 2.13%, while those of the control group were 10.64% and 19.15%, with statistically significant differences between groups (P<0.05). The difference of incidences of adverse reactions between the treatment of the observation group (0.00%) and the control group (2.13%) were not statistically significant (P>0.05). Conclusion Clinical effect of intravenous injection of gamma globulin combined with methylprednisolone in treating measles complicated with severe pneumonia treatment is better than single injection of gamma globulin. It is safe and of high clinical value.
