目的:探讨个体化现况-背景-评估-沟通(SBAR)沟通模式在骨科疼痛管理医护沟通中的应用。方法在骨科无痛病房实施以护士为主体的疼痛管理模式,设计SBAR疼痛评估沟通表,对骨三科12名经过培训的护士,采用个体化SBAR模式进行医护沟通管理。结果应用SBAR沟通模式后,因沟通不良引起的患者疼痛控制不良率由30%下降到3%;患者疼痛控制的满意度由92.7%提升到98.7%;医生对护士工作满意度由86%提升到97%;12名护士在应用SBAR沟通模式的体验高于使用模式前,差异有统计学意义(P<0.05)。结论结合科室特点的个体化SBAR沟通模式,能提高疼痛管理质量,提升护士体验,赢得满意度,促进医护和谐团队的建立,推动了患者安全文化的发展。

Objective To study the application of individualized situation‐background‐assessment‐recommendation (SABA) mode for pain management doctor and nurse communication in orthopedics department .Method 12 nurses in third orthopedics department was trained by self‐designed SABA communication table and implement pain man‐agement in painless orthopedics ward by nurses as the main body .Result The defective rate of patients pain control was reduced from 30% to 3% after the application of SABA communication mode .The patients’ pain control satis‐faction rate increased from 92 .7% to 98 .7% .The doctors’ satisfaction rate on nurses increased from 86% to 97% . The experience of communication among nurse was hihger than that before applying SABA mode .There was signifi‐cant difference before and after SABA mode application(P<0 .05) .Conclusion The individualized SBAR communica‐tion mode could improve the quality of pain management and the nurse’ experience win the satisfaction ,and promote the establishment of the medical team harmony and the development of the patient safety culture .
