目的 探讨血清β-羟丁酸(β-HB)和乳酸( LAC)水平变化在糖尿病酮症酸中毒( DKA)诊断中的临床价值.方法 在贝克曼AU2700全自动生化分析仪上对57例糖尿病非酮症患者( NDK组)、63例糖尿病酮症酸中毒患者( DKA组)、49例酮症混合性酸中毒患者( LDKA组)和65 例同期正常体检健康者(对照组)的血清样本进行 β-HB、LAC 和HbA1c检测,并对结果进行统计分析. 结果 NDK组检测血清β-HB、LAC和HbA1c水平高于对照组,DKA组检测血清β-HB、LAC和HbA1c水平明显高于NDK组,组间比较差异有统计学意义(P<0. 05);DKA组和LDKA组检测血清β-HB、LAC和HbA1c水平明显高于对照组,LDKA组检测血清β-HB、LAC和HbA1c水平明显高于NDK组,组间比较差异有统计学意义(P<0. 01);NDK组血清β-HB与LAC联合检测的阳性检出率高于单项阳性检出率,但差异无统计学意义(P>0. 05),DKA组和LDKA组血清 β-HB 与 LAC 联合检测的阳性检出率高于单项阳性检出率,差异有统计学意义(P <0. 05). 结论 联合检测血清β-HB和LAC有助于提高DKA诊断率.

Objective To explore the changed levels of serum β-hydroxybutyric acid(β-HB) and lactic acid ( LAC) in diagnosis of diabetic ketoacidosis( DKA) . Methods In the Backman AU2700 automatic biochemical analyzer, 57 patients with nonketotic diabetic patients( NDK group) ,63 cases of diabetic ketoacidosis ( DKA group) ,49 cases of mixed acidosis ketosis pa-tients (LDKA group) and 65 cases of normal healthy persons(control group) serumβ-HB,LAC and HbA1c were tested, and the test results were statistical analysised. Results Group NDK serum β-HB,LAC and HbA1c levels were higher than the control group,DKA group levels were significantly higher than that of NDK group,the difference was statistically significant(P<0. 05), DKA group and LDKA group levels were significantly higher than those in the control group,LDK group levels were significantly higher than that of NDK group,there were significant difference (P<0. 01). The joint test of NDK serumβ-HB and LAC positive rate was higher than that of single positive rate,the difference was not statistically significant(P>0. 05),the joint test of DKA group and LDKA group serum β-HB and t LAC he positive rate of was higher than that of single positive rate,the difference was statistically significant(P<0. 05). Conclusion Combined detection of serum β-HB and LAC might improved the diagnostic rate of DKA.
