目的 应用ROC曲线分析方法评价128层螺旋CT冠状动脉成像( CCTA)诊断急性冠脉综合征( ACS)的临床价值. 方法 回顾性分析临床疑似急性冠脉综合征的48例患者的冠状动脉CTA和冠状动脉造影( CAG)资料,以冠状动脉狭窄≥50%作为冠心病诊断标准,对结果采用ROC曲线分析. 结果 冠状动脉CTA诊断冠心病的敏感度92% ,特异度94%,阳性预测值88%,阴性预测值95%;CCTA和CAG结果呈高度相关性;CCTA的ROC曲线下面积为0. 945. 冠状动脉CTA诊断的斑块数量及心肌桥较CAG多. 结论 128层螺旋CT冠状动脉CTA诊断冠状动脉狭窄具有较高的准确性,且能识别斑块的危险程度.

Objective To assess the value of 128-SCT coronary CT angiography ( CCTA) in the diagnosis of acute coro-nary syndrome( CAG) with ROC curve. Methods Data of CTA and coronary angiography( CAG) in 48 patients with suspected ACS were evaluated by ROC curve analysis. Stenosis more than or equal to 50% of coronary artery was taken as the diagnostic sdandards of CHD. Results Compared with SCA,the sensitivity, specificity, positive predictive valueand negative predictive valueof CTA in diagnosing CHD was 92%, 94%,88% and 95%,respectively. There were no significant difference between CCTA and CAG. The area under curve of ROC of CTA was 0. 945. The plaques detected by CCTA and MB-MCA were more than CAG. Conclusion 128-SCT CCTA might has a good capabilities in identify different kinds of plaques and diagnose on narrow degree of coronary arter-y.
