目的：探讨局部枸橼酸抗凝在急性中毒患者血液灌流中的抗凝效果。方法：将37例拟行血液灌流的急性中毒患者随机分为普通肝素（SH）组、低分子量肝素（LMWH）组和局部枸橼酸钠（RCA）组，分别采用 SH、LMWH 及 RCA 抗凝，监测三组血小板计数、凝血活酶时间及灌流器凝血和身体出血情况。结果：RCA 组的灌流器凝血程度、出血发生率、凝血功能及血小板计数，与SH 组、LMWH 组比较差异均有统计学意义（P ﹤0.05）。结论：急性中毒患者血液灌流中使用 RCA 抗凝效果优于抗凝。
목적：탐토국부구연산항응재급성중독환자혈액관류중적항응효과。방법：장37례의행혈액관류적급성중독환자수궤분위보통간소（SH）조、저분자량간소（LMWH）조화국부구연산납（RCA）조，분별채용 SH、LMWH 급 RCA 항응，감측삼조혈소판계수、응혈활매시간급관류기응혈화신체출혈정황。결과：RCA 조적관류기응혈정도、출혈발생솔、응혈공능급혈소판계수，여SH 조、LMWH 조비교차이균유통계학의의（P ﹤0.05）。결론：급성중독환자혈액관류중사용 RCA 항응효과우우항응。
Objective:To investigate the anticoagulative effect of regional citrate anticoagulation for hemoperfusion in patients with acute poisoning. Methods:37 acute poisoning patients who would receive hemoperfusion were randomly divided into group A(anticoagulation with unfractionated heparin),group B(anticoagulation with low molecular weight heparin)and group C(anticoagulation with local sodium cit-rate). The count of platelet,activated partial thromboplastin time and coagulation in hemofilter and bleeding were monitored and evaluated for all cases. Results:The differences in the comparison of the blood coagulation,bleeding,coagulation function and platelet count in group C with group A and B were statistically significant(P ﹤ 0. 05). Conclusion:The effect of citrate might be better at anticoagulation than hep-arin in hemoperfusion for acute poisoning patients.
