目的：观察改良“工”字型高举平台法固定尿管在神经内科脑卒中患者中的应用效果。方法将本院神经内科收治的脑卒中且留置尿管的患者124例按入科顺序随机分成对照组和观察组各62例患者，对照组采用常规双“高举平台”法将尿管固定在大腿内侧上方1／3处，观察组在双“高举平台”法的基础上，改用“工”字型胶带固定尿管在下腹部。结果观察组发生胶带松脱或脱管3例（4．8％）及尿路损伤8例（12．9％），而对照组发生胶带松脱或脱管13例（21．0％）、尿路损伤22例（35．4％），2组差异有统计学意义（P ＜0．05）。结论神经内科脑卒中患者采用改良“工”字型高举平台法固定尿管，可有效减少胶带松脱和脱管的发生，减轻尿管牵拉疼痛，减轻患者痛苦。
목적：관찰개량“공”자형고거평태법고정뇨관재신경내과뇌졸중환자중적응용효과。방법장본원신경내과수치적뇌졸중차류치뇨관적환자124례안입과순서수궤분성대조조화관찰조각62례환자，대조조채용상규쌍“고거평태”법장뇨관고정재대퇴내측상방1／3처，관찰조재쌍“고거평태”법적기출상，개용“공”자형효대고정뇨관재하복부。결과관찰조발생효대송탈혹탈관3례（4．8％）급뇨로손상8례（12．9％），이대조조발생효대송탈혹탈관13례（21．0％）、뇨로손상22례（35．4％），2조차이유통계학의의（P ＜0．05）。결론신경내과뇌졸중환자채용개량“공”자형고거평태법고정뇨관，가유효감소효대송탈화탈관적발생，감경뇨관견랍동통，감경환자통고。
ABSTRACT:Objective To observe the effect of improved “工”type urinary catheter fixation in treatment of patients with stroke.Methods A total of 124 stroke patients with urinary catheter in the neurosurgery ward were randomly divided into control group and observation group,62 cases in each group.The urinary catheter in the control group was located in the one third of inner thighs (conventional “Raised-Platform”method),while the urinary catheter in the observation group was fixed on abdomen by using “工”type adhesive tape on the basis of the control group.Results Incidence of pipe -loose or extubation and urethral injury were 3 cases (4.8%)and 8 cases (12.9%)in the observation group,which were 13 cases (21.0%)and 22 cases(35.4%)in the control group,and there were significant differences between the two groups(P <0.05).Conclu-sion Application of improved “工”type urinary catheter fixation in treatment of stroke patients can effectively reduce the incidence of pipe-loose or extubation and urethral injury,reduce the stretch pain of urethral ducts and alleviate the sufferings of patients.
