目的:探讨在3.0 T MRI上运用IDEAL-IQ序列进行肝脏脂肪定量分析的可行性。方法对31例经临床确诊的脂肪肝患者及20名健康志愿者行常规MRI扫描及IDEAL-IQ序列检查;同期空腹采静脉血检测血清甘油三酯(TG)、总胆固醇(TC)及高密度脂蛋白胆固醇(HDL-C)含量;扫描前后2 d内行上腹部CT扫描测量肝脾CT值并计算比值。比较经IDEAL-IQ直接测得肝脏脂肪分量值、肝脾CT值比值及血清学检查各项指标的一致性。结果健康对照组与脂肪肝组的TG、TC及HDL-C值差异有统计学意义(P<0.05);健康对照组与脂肪肝组的肝脾CT值比值差异有统计学意义(P<0.05);健康对照组与脂肪肝组的脂肪分量值差异有统计学意义(P<0.05);脂肪肝组的脂肪分量值与血清TG、TC值呈显著正相关(r=0.836、0.852,P<0.05),而与肝脾CT值比值、HDL-C值呈负相关(r=-0.992、-0.735,P<0.05)。结论 IDEAL-IQ成像技术定量评估肝脏的脂肪含量是可行的,并与肝脾CT值比值及临床血清学检测之间具有良好的相关性,对脂肪肝的定量诊断具有重要的临床应用价值。

목적:탐토재3.0 T MRI상운용IDEAL-IQ서렬진행간장지방정량분석적가행성。방법대31례경림상학진적지방간환자급20명건강지원자행상규MRI소묘급IDEAL-IQ서렬검사;동기공복채정맥혈검측혈청감유삼지(TG)、총담고순(TC)급고밀도지단백담고순(HDL-C)함량;소묘전후2 d내행상복부CT소묘측량간비CT치병계산비치。비교경IDEAL-IQ직접측득간장지방분량치、간비CT치비치급혈청학검사각항지표적일치성。결과건강대조조여지방간조적TG、TC급HDL-C치차이유통계학의의(P<0.05);건강대조조여지방간조적간비CT치비치차이유통계학의의(P<0.05);건강대조조여지방간조적지방분량치차이유통계학의의(P<0.05);지방간조적지방분량치여혈청TG、TC치정현저정상관(r=0.836、0.852,P<0.05),이여간비CT치비치、HDL-C치정부상관(r=-0.992、-0.735,P<0.05)。결론 IDEAL-IQ성상기술정량평고간장적지방함량시가행적,병여간비CT치비치급림상혈청학검측지간구유량호적상관성,대지방간적정량진단구유중요적림상응용개치。
Objective To observe the feasibility of liver fat quantification using iterative decomposition of water and fat with echo asymmetry and least-squares estimation (IDEAL)-Image Quantification (IQ) gradient multi-echo sequence imaging at 3.0 T MR.Methods A total of 31 patients with fatty liver and 20 healthy volunteers underwent conventional MRI and IDEAL-IQ sequence analysis. The fasting venous blood lipid (TG, TC, HDL-C) were tested on the same day. The ratio value of liver and spleen CT value were measured on abdominal CT scanning before and after the two days. The relative fat component was measured and compared with the results of the blood lipid and L/S CT value.Results There were significant differences in TG, TC, HDL-C level, L/S CT value and fat component between the two groups. There were significant positive correlation between the fat component and TG, TC level (r=0.836, 0.852,P<0.05), and significant negative correlation between the fat component and the L/S CT value and HDL-C level in the fatty liver group (r=-0.992, -0.735,P<0.05).Conclusion It is feasible to use IDEAL-IQ imaging at 3.0 T MR. There was a good correlation between L/S CT value and clinical serological detection. It has important clinical value of fatty liver disease quantitatively.
