目的 探讨缺血后处理(IPO)对梗阻性黄疸(OJ)大鼠肾缺血再灌注损伤后肾组织丙二醛(MDA)、超氧化物歧化酶( SOD)水平的影响. 方法 SD大鼠100只,按随机数字表法分成5组:正常对照组( Control-Sham组)、正常肾缺血再灌注组( Control-I/R组) ,梗阻性黄疸组( OJ-Sham组)、梗阻性黄疸+肾缺血再灌注组( OJ-I/R组) ,梗阻性黄疸+肾缺血再灌注+IPO组( OJ-I/R+IPO组) ,每组20只. 根据缺血后再灌注0 h、1 h、3 h、6 h时间点(记为T0、T1、T2、T3 ) ,各组又分为4个亚组各5只. 在梗阻性黄疸模型制作成功1周后,建立肾缺血再灌注模型. 分别于T0、T1、T2、T3 检测各组大鼠血清肌酐( Scr)、尿素氮( BUN )和肾组织 MDA、SOD 水平. 结果 与 Control-Sham 组比较, T0、T1、T2、T3 时间点胆道梗阻的各组(OJ-Sham组、OJ-I/R组、OJ-I/R+IPO组),BUN、Scr和MDA明显升高(P<0.05),SOD降低(P<0.05). 和OJ-I/R组比较, OJ-I/R+IPO组在T1、T2、T3 时间点BUN、Scr和MDA均明显降低(P<0.05),SOD升高(P<0.05),T2 时间点病理显示,肾小球血管扩张明显减轻,肾小管上皮细胞形态及排列趋于正常. 结论 IPO可以减轻OJ大鼠的肾缺血再灌注损伤,其机制可能与减轻氧化应激反应有关.

목적 탐토결혈후처리(IPO)대경조성황달(OJ)대서신결혈재관주손상후신조직병이철(MDA)、초양화물기화매( SOD)수평적영향. 방법 SD대서100지,안수궤수자표법분성5조:정상대조조( Control-Sham조)、정상신결혈재관주조( Control-I/R조) ,경조성황달조( OJ-Sham조)、경조성황달+신결혈재관주조( OJ-I/R조) ,경조성황달+신결혈재관주+IPO조( OJ-I/R+IPO조) ,매조20지. 근거결혈후재관주0 h、1 h、3 h、6 h시간점(기위T0、T1、T2、T3 ) ,각조우분위4개아조각5지. 재경조성황달모형제작성공1주후,건립신결혈재관주모형. 분별우T0、T1、T2、T3 검측각조대서혈청기항( Scr)、뇨소담( BUN )화신조직 MDA、SOD 수평. 결과 여 Control-Sham 조비교, T0、T1、T2、T3 시간점담도경조적각조(OJ-Sham조、OJ-I/R조、OJ-I/R+IPO조),BUN、Scr화MDA명현승고(P<0.05),SOD강저(P<0.05). 화OJ-I/R조비교, OJ-I/R+IPO조재T1、T2、T3 시간점BUN、Scr화MDA균명현강저(P<0.05),SOD승고(P<0.05),T2 시간점병리현시,신소구혈관확장명현감경,신소관상피세포형태급배렬추우정상. 결론 IPO가이감경OJ대서적신결혈재관주손상,기궤제가능여감경양화응격반응유관.
Objective To explore the effect of ischemic postconditioning(IPO) on the levels of malondialdehyde(MDA) and superoxide dismutase( SOD) of renal tissues in rats with obstructive jaundice ( OJ) combined with renal ischemia-reperfusion injury. Methods One hundred SD rats were randomly divided into 5 groups including normal control group(Control-Sham group),normal renal ischemia-reperfusion group(Control-I/R group),obstructive jaundice group(OJ-Sham group),obstructive jaundice and renal ischemia-reperfusion group(OJ-I/R group),obstructive jaundice combined with renal ischemia-reperfusion and IPO group(OJ-I/R+IPO group),20 cases in each group.According to 0,1,3 and 6 hours after ischemia-reperfusion(marked as T0,T1,T2 and T3,respectively),each group was divided into four subgroups(n=5).The renal ischemia-reperfusion model was developed one week after the establishment of OJ model.The levels of serum creatinine(Scr) and blood urea nitrogen(BUN) as well as the levels of MDA and SOD of renal tissues were detected at T0,T1,T2 and T3, respectively.Results OJ-Sham group,OJ-I/R group and OJI-/R+IPO group gained significant higher levels of BUN ,Scr and MDA,and lower levels of SOD at T 0,T 1,T 2 and T 3 compared with Control-Sham group( P<0.05).The levels of BUN, Scr and MDA decreased significantly,and the levels of SOD increased at T1 , T2 and T3 in the OJ-I/R+IPO group in contrast with those in the OJ-I/R group (P<0.05),and the pathological result showed that glomerular vascular dilatation reduced significantly,and the morphology and alignment of renal tubular epithelial cells tended to be normal at T2 in the OJ-I/R+IPO group.Conclusion IPO can alleviate renal ischemia-reperfusion injury in OJ rats,and its mechanism might be related to the reduction of oxidative stress.
