目的:探讨急性冠脉综合征(acute coronary syndrome,ACS)患者血清缺血修饰白蛋白(ischemia-modified albumin,IMA)水平与冠脉病变程度的关系。方法入选2013年12月~2014年12月间于濮阳市安阳地区医院心血管科收住院的160例急性冠脉综合征患者,检测其血清IMA水平,同时行冠脉造影检查并分析结果。根据造影结果分为两组,B组(无冠脉狭窄或冠脉狭窄<50%者)42例;C组:冠脉狭窄>50%者118例。选取30例健康体检者(A组),检测其IMA水平作对照组。对C组(冠脉狭窄>50%患者)进行亚组分析,据冠脉病变形态特点分简单病变组和复杂病变组;据冠脉Gensini积分分为三组:C1组积分<20分,C2组20~40分、C3组>40分。结果 C组血清IMA水平显著高于A组及B组(P<0.05),C组亚组分析显示,随着冠脉病变程度的加重,IMA水平增加。结论 IMA水平升高与ACS患者冠脉病变严重程度有较高一致性,可做为ACS早期诊断和严重程度的参考指标。

목적:탐토급성관맥종합정(acute coronary syndrome,ACS)환자혈청결혈수식백단백(ischemia-modified albumin,IMA)수평여관맥병변정도적관계。방법입선2013년12월~2014년12월간우복양시안양지구의원심혈관과수주원적160례급성관맥종합정환자,검측기혈청IMA수평,동시행관맥조영검사병분석결과。근거조영결과분위량조,B조(무관맥협착혹관맥협착<50%자)42례;C조:관맥협착>50%자118례。선취30례건강체검자(A조),검측기IMA수평작대조조。대C조(관맥협착>50%환자)진행아조분석,거관맥병변형태특점분간단병변조화복잡병변조;거관맥Gensini적분분위삼조:C1조적분<20분,C2조20~40분、C3조>40분。결과 C조혈청IMA수평현저고우A조급B조(P<0.05),C조아조분석현시,수착관맥병변정도적가중,IMA수평증가。결론 IMA수평승고여ACS환자관맥병변엄중정도유교고일치성,가주위ACS조기진단화엄중정도적삼고지표。
Objective To investigate the relationship between level of serum ischemia-modified albumin (IMA) and severity of coronary artery disease in patients with acute coronary syndrome (ACS).Methods The patients with ACS (n=160) were chosen and all of them were given detection of IMA level and coronary angiography (CAG). According to CAG outcomes the patients were divided into group B (without coronary artery stenosis or coronary stenosis<50%,n=42) and group C (coronary artery stenosis>50%,n=118). Other 30 healthy people were chosen and given detection of IMA level (group A). Group C was divided again, according to lesion features, into simple group and complex group, and according to Gensini integral, into group C1 (Gensini integral<20), group C2 (Gensini integral from 20 to 40) and group C3 (Gensini integral>40).Results The level of serum IMA was significantly higher in group C than that in group A and group B (P<0.05). The level of serum IMA increased as coronary lesion exacerbated.Conclusion The increase of serum IMA level is consistent to the severity of coronary artery lesion, which can be taken as a reference index for early diagnosis of ACS.
