目的 探讨应用腹部"H"形真皮下血管网瓦合皮瓣修复多指皮肤套脱伤的方法和临床效果.方法 对41例113指近中节以远皮肤套脱伤患者,采用腹部"H"形真皮下血管网瓦合皮瓣进行修复.结果 41例患者术后随访3个月至2.5年,平均9个月.皮瓣均存活,外形、质地良好.患者对外观比较满意.基本恢复抓握、持物等功能,皮瓣感觉为S2～S3.根据中华医学会手外科学会手功能评定试用标准综合评价:优21例,良13例,差7例;优良率为83％.对供区没有影响.结论 采用腹部"H"形真皮下血管网瓦合皮瓣修复多指皮肤套脱伤,手术操作简单,疗效确切,具有较高的临床价值.

Objective To investigate the surgical methods and clinical results of transferring the H shape abdominal tile-coupling flap with subdermal vascular network to repair multi-finger degloving injuries.Methods Forty-one cases (113 fingers) of degloving injury of the fingers distal to the proximal interphalangeal joint were treated with H shape abdominal tile-couphng flaps with subdermal vascular network.Early functional exercises were carried out postoperatively.Results All 41 cases were follow-up for 3 months to 2.5 years (average, 9 months).All the flaps survived uneventfully.The texture and appearance of the flaps were satisfactory.Grasp and pinch function was basically restored.Sensation of the flaps returned to S2 to S3.According to the criteria of Chinese Hand Surgery Society, the functions of these fingers were rated as good in 21 cases, fair in 13 cases and poor in 7 cases.The overall excellent and good rate was 83％.There was no morbidity at flap donor site.Conclusion The H shape abdominal tile-coupling flap with subdermal vascular network is a simple and reliable procedure to repair multi-finger degloving injuries.
