目的:探讨预防颈动脉斑块形成的血清高密度脂蛋白胆固醇(HDL-C)的目标值。方法收集2012年1月~2013年12月在北京小汤山医院进行健康体检12883例的资料,按照颈动脉超声表现,分为颈动脉未见明显异常组(7585例)、内中膜增厚组(886例)、斑块形成组(4412例)。对不同性别的各组患者血清HDL-C水平进行统计学分析,从预防颈动脉斑块形成的角度,推导HDL-C的目标值。结果颈动脉未见明显异常组、内中膜增厚组、斑块形成组男性的HDL-C水平分别为(1.19±0.27)、(1.17±0.26)、(1.16±0.27)mmol/L;颈动脉未见明显异常组、内中膜增厚组、斑块形成组女性的HDL-C水平分别为(1.48±0.35)、(1.42±0.33)、(1.40±0.34)mmol/L,相同性别的三组间HDL-C水平比较,差异有高度统计学意义(P<0.01);相同性别的颈动脉未见明显异常组与内中膜增厚组、斑块形成组的HDL-C水平比较,差异有高度统计学意义(P<0.01);男性内中膜增厚组与斑块形成组的HDL-C水平比较,差异有统计学意义(P<0.05);但女性内中膜增厚组与斑块形成组的HDL-C水平比较,差异无统计学意义(P>0.05)。结论相同性别不同颈动脉超声表现之间HDL-C水平有差别。斑块形成的患者血清HDL-C水平低于未形成斑块。为预防颈动脉斑块形成,中国男性的HDL-C的目标水平应跃1.17 mmol/L,女性应跃1.40 mmol/L。

목적:탐토예방경동맥반괴형성적혈청고밀도지단백담고순(HDL-C)적목표치。방법수집2012년1월~2013년12월재북경소탕산의원진행건강체검12883례적자료,안조경동맥초성표현,분위경동맥미견명현이상조(7585례)、내중막증후조(886례)、반괴형성조(4412례)。대불동성별적각조환자혈청HDL-C수평진행통계학분석,종예방경동맥반괴형성적각도,추도HDL-C적목표치。결과경동맥미견명현이상조、내중막증후조、반괴형성조남성적HDL-C수평분별위(1.19±0.27)、(1.17±0.26)、(1.16±0.27)mmol/L;경동맥미견명현이상조、내중막증후조、반괴형성조녀성적HDL-C수평분별위(1.48±0.35)、(1.42±0.33)、(1.40±0.34)mmol/L,상동성별적삼조간HDL-C수평비교,차이유고도통계학의의(P<0.01);상동성별적경동맥미견명현이상조여내중막증후조、반괴형성조적HDL-C수평비교,차이유고도통계학의의(P<0.01);남성내중막증후조여반괴형성조적HDL-C수평비교,차이유통계학의의(P<0.05);단녀성내중막증후조여반괴형성조적HDL-C수평비교,차이무통계학의의(P>0.05)。결론상동성별불동경동맥초성표현지간HDL-C수평유차별。반괴형성적환자혈청HDL-C수평저우미형성반괴。위예방경동맥반괴형성,중국남성적HDL-C적목표수평응약1.17 mmol/L,녀성응약1.40 mmol/L。
Objective Explore the prevention of serum high density lipoprotein cholesterol in the formation of carptid plaque (HDL-C) target value. Methods From January 2012 to December 2013, in Beijing Xiaotangshan Hospital, the data of 12 883 cases from health examination were collected, according to carotid artery ultrasound, they were divided into carotid artery without obvious abnormal group (7585 cases), intima-media thickness group (886 cases), plaque for-mation group (4412 cases). The differences on the level of serum HDL-C in men and women from three groups were analyzed, from the perspective of prevention of carotid artery plaque formation, the HDL-C target values was infer-enced. Results HDL-C levels of men in the three groups were (1.19±0.27), (1.17±0.26), (1.16±0.27) mmol/L; HDL-C levels of women in the three groups were (1.48±0.35), (1.42±0.33), (1.40±0.34) mmol/L. The HDL-C level of same gen-der in three groups were compared, the difference was statistically significant (P<0.01); the HDL-C level of same gen-der in carotid artery without obvious abnormal group were compared with those of same gender in intima-media thick-ness group and plaque formation group, the differences were statistically significant (P<0.01);the HDL-C level of men in intima-media thickness group was compared with that in plaque formation group, the difference was statistically sig-nificant (P<0.05);but HDL-C level of women in intima-media thickness group was compared with that in plaque for-mation group, the difference was not statistically significant (P>0.05). Conclusion The HDL-C level of the same gen-der in different carotid artery ultrasound have differences. HDL-C level of patients with plaque formation is lower than that of patients without plaque formation, the HDL-C target level of Chinese men should be >1.17 mmol/L, and that of women should be>1.40 mmol/L.
