目的：探讨提上睑肌缩短术治疗重度上睑下垂的疗效。方法连续收集2008年3月至2014年12月因重度上睑下垂在莒南县光明医院就诊的患者47例（63只眼），按手术方式的不同分为A、B两组，A组20例（30只眼）患者行额肌筋膜瓣悬吊术， B组27例（33只眼）患者行提上睑肌缩短术。两组均为同一医师手术，术后随访1年，观察两组术后功能及外观疗效。结果 A、B两组患者术后早期成功率均为99％，术后1年矫正满意率：A组基本满意26只眼（86．7％），欠矫3只眼（10％），过矫1只眼（3．3％）。 B组为基本满意30只眼（90．9％），欠矫2只眼（6．1％），复发1只眼（3％），两组从上睑高度方面比较差异无统计学意义（ P ＞0．05）。重睑形成满意率：A组，基本满意20只眼（66．7％），良好9只眼（30％），不满意1只眼（3．3％）。 B组为基本满意30只眼（90．9％），良好2只眼（6．1％），不满意1只眼（3％），两组从重睑形成方面比较差异有统计学意义（ P ＜0．05）。结论提上睑肌缩短术可有效的矫正重度上睑下垂，且重睑形成良好，手术效果稳定。
목적：탐토제상검기축단술치료중도상검하수적료효。방법련속수집2008년3월지2014년12월인중도상검하수재거남현광명의원취진적환자47례（63지안），안수술방식적불동분위A、B량조，A조20례（30지안）환자행액기근막판현조술， B조27례（33지안）환자행제상검기축단술。량조균위동일의사수술，술후수방1년，관찰량조술후공능급외관료효。결과 A、B량조환자술후조기성공솔균위99％，술후1년교정만의솔：A조기본만의26지안（86．7％），흠교3지안（10％），과교1지안（3．3％）。 B조위기본만의30지안（90．9％），흠교2지안（6．1％），복발1지안（3％），량조종상검고도방면비교차이무통계학의의（ P ＞0．05）。중검형성만의솔：A조，기본만의20지안（66．7％），량호9지안（30％），불만의1지안（3．3％）。 B조위기본만의30지안（90．9％），량호2지안（6．1％），불만의1지안（3％），량조종중검형성방면비교차이유통계학의의（ P ＜0．05）。결론제상검기축단술가유효적교정중도상검하수，차중검형성량호，수술효과은정。
Objective To explore the efficacy of levator muscle resection in severe blepharoptosis.Methods Sixty-three eyes of 47 patients with severe blepharoptosis were enrolled from March 2008 to December 2014 in the bright hospital of Junan Red Cross Society.Patients were divided into two groups according to the operation method.Group A (20 patients, 30 eyes) was treated with frontalis muscle flap suspension, and group B (27 patients, 33 eyes) was treated with levator muscle resection.All the operations were done by the same physician.All patients were followed up for 1 year and the curative effects were compared between the two groups.Results Operation success rate was 99%in both groups.At 1 year, results maintained satisfied in 26 eyes from group A.The cure rate was 86.7%.Undercorrection (3 eyes, 10%) and overcorrection (1 eye, 3.3%) were noticed.In group B, results remained satisfied in 30 eyes, and the cure rate was 90.9%.There were undercorrection (2 eyes, 6.1%) and recurrence (1 eye, 3%).There was no significant different in“lid height” grading between the two group ( P >0.05).In“lid crease” grading, there were 20 cases of basically satis-factory (66.7%), 9 cases of good (30%), and 1 case of unsatisfactory (3.3%) in group A.In group B, the numbers were 30 cases (90.9%), 2 cases (6.1%) and 1 case (3%), respectively.There was significant different in “lid crease” grading between the two group ( P <0.05).Conclusion Lavator muscle resection has good effect on the treat-ment of severe blepharoptosis.It resulted in good“lid crease” and the effect was reliable .
