目的 探讨组织上皮-钙粘连素(CDH1)基因异常甲基化和血清人附睾分泌蛋白4 (HE4)检测对卵巢癌和卵巢内异症囊肿的鉴别价值.方法 选取卵巢癌患者(卵巢癌组) 38例、卵巢内异症囊肿患者(内异症组) 41例,另选取子宫或卵巢良性病变患者42例(对照组),采用甲基化聚合酶联反应技术检测CDH1基因启动子区甲基化状态,采用酶联免疫吸附试验检测血清HE4水平.比较以上指标的变化.结果 CDH1 DNA在内异症组、卵巢癌组及对照组中的甲基化表达率分别为4.8%(2/41)、39.4%(15/38)、2.3%(1/42).内异症组的CDH1 DNA甲基化表达率与对照组比较差异无统计学意义(χ2=0.37,P=0.542),卵巢癌组的CDH1基因甲基化表达率与对照组比较差异有统计学意义(χ2=11.24,P=0.001),卵巢癌组的CDH1基因甲基化表达率与内异症组比较差异有统计学意义(χ2=13.79,P=0.000).血清HE4在内异症组、卵巢癌组及对照组水平分别为52.7 pmol/L、537.2 pmol/L、50.3 pmol/L,内异症组血清HE4水平与对照组比较差异无统计学意义(t=0.63,P=0.532),卵巢癌组的血清HE4水平与对照组比较显著增高,差异有统计学意义(t=54.27,P=0.000),卵巢癌组与内异症组比较显著增高,差异有统计学意义(t=55.33,P=0.000).卵巢癌组CDH1基因甲基化DNA表达量与血清HE4水平具有相关性(r=0.527,P<0.01),卵巢癌组CDH1甲基化DNA表达量、血清HE4水平分别与CA125水平具有相关性(r=0.539、0.520,P<0.01).内异症组血清HE4水平与CA125水平无明显相关性(r=0.127,P>0.05).结论 联合检测组织CDH1 DNA甲基化的发生和血清HE4水平可应用于卵巢内异症囊肿和卵巢癌的鉴别诊断,提高两种疾病的诊断率.

Objective To investigate the abnormal methylation of CDH1 gene and the detection of serum hu-man epididymis protein 4 (HE4) in the identification of ovarian cancer and ovarian endometriosis cyst. Methods Thir-ty-eight patients with ovarian cancer (group A), 41 patients with ovarian ectopic cyst (group B) and 42 patients with be-nign uterine or ovarian lesion (the control group) were enrolled in the study. The methylation status of CDH1 gene pro-moter region was detected by methylation-specific polymerase chain reaction, and the levels of serum HE4 were de-tected with enzyme-linked immunosorbent assay. The changes of the above indicators were compared. Results The expression rates of CDH1 DNA methylation in group B, group A, control group were 4.8% (2/41), 39.4% (15/38), 2.3%(1/42), respectively, with no statistically significant difference between group B and the control group (χ2=0.37, P=0.542), but statistically significant difference between group A and the control group (χ2=11.24, P=0.001), as well as between group A and group B (χ2=13.79, P=0.000). The levels of serum HE4 in group B, group A and control group were 52.7 pmol/L, 537.2 pmol/L, 50.3 pmol/L, respectively, with no statistically significant difference between group B and the control group (t=0.63, P=0.532), but statistically significant difference between group A and the control group (t=54.27, P=0.000), as well as between group A and group B (t=55.33, P=0.000). In group A, the DNA expres-sion quantity of CDH1 gene methylation and levels of serum HE4 are correlated (r=0.527, P<0.01), which are respec-tively correlated to CA125 levels (t=0.539, 0.520, P<0.01). In group B, the levels of serum HE4 had no correlation with CA125 levels (r=0.127, P>0.05). Conclusion Joint detection of CDH1 gene methylation and serum HE4 levels can be applied to the differential diagnosis between ovarian endometriosis cyst and ovarian cancer, which improves the diagnosis rate of the two kinds of diseases.
