目的：观察锁定钢板外固定术联合局部负压吸引治疗儿童下肢开放性骨折的效果。方法下肢开放性骨折患儿36例，年龄5～14岁。采用锁定钢板外固定术联合负压吸引治疗。术后3～8个月根据X线检查结果评估骨折愈合情况，去除外固定锁定板。按照Flynn骨折愈合评判标准评价疗效。结果36例均获得随访，骨愈合时间3～7个月，钢板拆除时间4～8个月。创面一期愈合8例，延期缝合8例，二期植皮20例。本组疗效优28例、良7例、差1例，优良率为97．2％。3例患儿骨折延迟愈合；1例骨折愈合好，但因车碾压伤致肌肉、皮肤毁损严重，仍有膝关节功能受限；1例患儿过早下地活动，术后5个月胫骨成角畸形愈合（＞10°）。本组无骨折不愈合、再骨折等并发症。结论锁定钢板外固定术联合局部负压吸引治疗儿童下肢开放性骨折效果较好。
목적：관찰쇄정강판외고정술연합국부부압흡인치료인동하지개방성골절적효과。방법하지개방성골절환인36례，년령5～14세。채용쇄정강판외고정술연합부압흡인치료。술후3～8개월근거X선검사결과평고골절유합정황，거제외고정쇄정판。안조Flynn골절유합평판표준평개료효。결과36례균획득수방，골유합시간3～7개월，강판탁제시간4～8개월。창면일기유합8례，연기봉합8례，이기식피20례。본조료효우28례、량7례、차1례，우량솔위97．2％。3례환인골절연지유합；1례골절유합호，단인차년압상치기육、피부훼손엄중，잉유슬관절공능수한；1례환인과조하지활동，술후5개월경골성각기형유합（＞10°）。본조무골절불유합、재골절등병발증。결론쇄정강판외고정술연합국부부압흡인치료인동하지개방성골절효과교호。
Objective To observe the effect of locking plate external fixation combined with topieal negative pressure drainage in treatment of open fractures in lower limbs of children .Methods Thirty-six children ( aged 5-14 years ) with open fractures in lower limbs were selected .The locking plate external fixation combined with negative pressure drainage was used in the treatment .Three to eight months after surgery , the healing of the fracture was evaluated based on the ima-ging data, and then the locking plate external fixation was removed .The efficacy was analyzed in accordance with the Flynn fracture healing criteria.Results All 36 patients were followed up.The bone healing time was 3-7 months.The locking plate was removed within 4-8 months.Primary healing was found in 8 cases, 8 cases of second intention and 20 cases nee-ded skin grafting by second intention .According to the Flynn fracture healing criteria , 28 cases were excellent , 7 cases were good, one was poor , and the excellent and good rate was 97.2%.Three pediatric fractures had delayed union .One had limited knee joint function due to the injury of muscle and skin damage caused by vehicle rolling .One case was dis-charged without timely review , due to early ambulation , the tibia deformity healing >10°was found in 5 months.There were no fracture non-union, re-fracture and other complications in this group .Conclusion The effect of locking plate ex-ternal fixation combined with topical negative pressure drainage in treatment of open fractures in lower limbs of children is good.
