目的：探讨分析影响乳腺癌哨兵淋巴结核素显像结果的各种因素。方法选择不同部位Ⅰ～Ⅱa期乳腺恶性肿瘤129例，其中瘤体局部切除手术患者17例，分析注射部位、显像剂、显像时间等不同因素对哨兵淋巴结显像结果的影响。结果99mTc-DX和99mTc-SC哨兵显像的阳性率差异无统计学意义（P＞0．5）。但两者哨兵淋巴结显像时间差异有统计学意义（P＜0．005）。行乳腺肿瘤局部切除术的患者哨兵淋巴结显像的阳性率低于未行手术的患者，但差异无统计学意义（P＞0．05）。不同的注射部位哨兵淋巴显像的阳性率各不相同，差异有统计学意义（P＜0．01）。不同位置的肿瘤哨兵淋巴显像阳性率各不相同，但差异无统计学意义（P＞0．25）。不同位置内乳淋巴阳性率各不相同，差异有统计学意义（ P＜0．005）。结论影响乳腺哨兵淋巴显像的因素较多，不同部位注射哨兵淋巴显像率各不相同，以乳晕区注射最高；99m Tc-DX较99m Tc-SC早；局部切除术对哨兵淋巴显像率无明显影响；不同部位肿瘤哨兵淋巴显像率各不相同，都表现为腋前区最高，中央区及内侧象限内乳淋巴显像率明显高于外侧象限。
목적：탐토분석영향유선암초병림파결핵소현상결과적각충인소。방법선택불동부위Ⅰ～Ⅱa기유선악성종류129례，기중류체국부절제수술환자17례，분석주사부위、현상제、현상시간등불동인소대초병림파결현상결과적영향。결과99mTc-DX화99mTc-SC초병현상적양성솔차이무통계학의의（P＞0．5）。단량자초병림파결현상시간차이유통계학의의（P＜0．005）。행유선종류국부절제술적환자초병림파결현상적양성솔저우미행수술적환자，단차이무통계학의의（P＞0．05）。불동적주사부위초병림파현상적양성솔각불상동，차이유통계학의의（P＜0．01）。불동위치적종류초병림파현상양성솔각불상동，단차이무통계학의의（P＞0．25）。불동위치내유림파양성솔각불상동，차이유통계학의의（ P＜0．005）。결론영향유선초병림파현상적인소교다，불동부위주사초병림파현상솔각불상동，이유훈구주사최고；99m Tc-DX교99m Tc-SC조；국부절제술대초병림파현상솔무명현영향；불동부위종류초병림파현상솔각불상동，도표현위액전구최고，중앙구급내측상한내유림파현상솔명현고우외측상한。
Objective To investigate the influence factors of radionuclide imaging of breast cancer sentinel lymph nodes.Methods 129 cases of patients with Ⅰ~Ⅱa stage breast malignant tumor were selected ,including 17 cases of patients underwent partial resection of the tumor .The injection site ,imaging agent ,developing time ,such as the influence of different fac-tors on the imaging results of the sentinel lymph node were analyzed .Results There had no statistically significant difference in positive rate of 99mTc-DX and 99mTc-SC sentinel imaging (P>0.5).But sentinel imaging time had statistically significant differ-ence (P<0.005).The positive rate of sentinel lymph node imaging in patients treated with local resection was lower than those received no surgery,but there had no statistically significant difference (P>0.05).The positive rate of sentinel imaging by differ-ent injection site was not identical, the difference was statistically significant (P<0.01).The positive rate of sentinel imaging with different tumor location was not identical , but there had no statistically significant difference (P>0.25).The positive rate of sentinel imaging with different location of internal mammary lymph node was not identical ,the difference was statistically signifi-cant (P<0.005).Conclusion The influencing factors of mammary sentinel lymphatic imaging is plenty .The positive rate in different injection parts are not identical ,and that of areola area is the highest;99m Tc-DX imaging is earlier than 99m Tc-SC ima-ging;Local resection has no obvious effect on the sentinel lymphatic imaging rate ;The positive rate of different parts of tumor are not identical ,the sentinel lymphatic imaging is the highest characterized by the area before the axillary , the central and medial quadrant internal mammary lymph node imaging rate is significantly higher than the lateral quadrant .
