目的 观察术前使用地塞米松对四肢骨折术后镇痛的影响.方法 将52例骨科拟行四肢骨折手术患者随机均分为地塞米松组和对照组,各26例.麻醉前30 min分别静脉注射地塞米松和生理盐水,2组均以舒芬太尼联合曲马多进行术后镇痛,观察术后5、10、15、20、40 h行视觉模拟量表(VAS)评分、术后恶心呕吐(PONV)评分、术后使用曲马多和胃复安病例数.结果 2组患者在术后20h VAS评分为最高(P<0.05);地塞米松组VAS和PONV评分在各时间点明显低于对照组(P<0.05);地塞米松组患者术后使用曲马多比例明显低于对照组(P<0.05).结论 四肢骨折术前给予地塞米松可增强术后镇痛效果.

목적 관찰술전사용지새미송대사지골절술후진통적영향.방법 장52례골과의행사지골절수술환자수궤균분위지새미송조화대조조,각26례.마취전30 min분별정맥주사지새미송화생리염수,2조균이서분태니연합곡마다진행술후진통,관찰술후5、10、15、20、40 h행시각모의량표(VAS)평분、술후악심구토(PONV)평분、술후사용곡마다화위복안병례수.결과 2조환자재술후20h VAS평분위최고(P<0.05);지새미송조VAS화PONV평분재각시간점명현저우대조조(P<0.05);지새미송조환자술후사용곡마다비례명현저우대조조(P<0.05).결론 사지골절술전급여지새미송가증강술후진통효과.
Objective To observe the effects of dexamethasone used preoperative for postoperative analgesia on limb fracture.Methods 52 termed limb fracture patients for postoperative analgesia were randomly divided into dexamethasone group (n=26) and contol group(n=26). Dexamethasone and saline were intravenous injection of respectively in dexamethasone group and contol group 30 min before anesthesia, and the same method that Sufentanil combined with Tramado for postoperative analgesia are used in the two groups. Some indexs were observed in the groups such as VAS and PONV score at each time, and the number of cases that used tramadol and metoclopramide postoperative.Results VAS score in the two groups of patients was the highest at 20 hours after the operation (P<0.05); VAS and PONV scores at each time point was signiifcantly lower in dexamethasone group than contol group (P<0.05); The proportion in dexamethasone group was obviously less than contol group that used tramadol and metoclopramide postoperative tramadol (P<0.05).Conclusion It can enhanced the effect of analgesic and reduces the degree of nausea and vomiting postoperative that patients were intravenous injected dexamethasone preoperative.
