目的:探讨社区重性精神疾病患者实施个案管理与基础管理的效果差异. 方法:选取40例重性精神疾病患者,随机分为个案组和对照组,每组各20例. 个案组结合患者情况制定个性化方案,予以综合干预措施;对照组仅维持基础管理. 采用阳性与阴性症状量表、日常生活能力量表、社会功能缺陷量表评定治疗效果. 结果:入组前两组SDSS、ADL及PANSS评分差异比较均无统计学意义(P>0. 05);经过一年的治疗后,两组的评分均低于治疗前,且SDSS和PANSS评分组间比较具有统计学意义(P<0. 05). 结论:在社区开展个案管理能够有效地改善患者的精神症状,提高社会功能,促进病情的好转.

Objective:To investigate difference of effects between case management and basic management in community pa-tients with severe psychosis. Methods:40 patients with severe psychosis from community were randomly divided into case management group (20 cases) and control group (20 cases). Case management group were given a personalized treatment and comprehensive inter-vention measures, whereas control group only maintained the basic management. Positive and negative syndrome scale ( PANSS) , ac-tivity of daily living scale ( ADL) , and social disability screening schedule ( SDSS) were used to assess the curative effects. Results:No significant differences were found at baseline in the scores of SDSS, ADL and PANSS between the two groups (P>0. 05);Howev-er, after a year of treatment, the scores of the two groups were lower than those before the treatment, and there were significant differ-ences in the scores of SDSS and PANSS between the two groups (P<0. 05). Conclusions: In the community, the implementation of case management can effectively improve the patients′mental symptoms, improve the social function, and promote condition improved.
