目的：探讨抑癌基因TCF21及凋亡相关基因Bax在非小细胞肺癌（NSCLC）中的表达及临床意义。方法收集广西宾阳县人民医院2013～2015年胸外科切除的NSCLC标本共44例（含癌及癌旁）。采用免疫组织化学方法（ABC法）检测TCF21、Bax在肺癌组织及正常组织中的表达，分析NSCLC组织中TCF21、Bax的表达及与临床病理特征的关系。结果在肺癌组织及癌旁组织中，TCF21的阳性率为34.09%vs.56.81%，差异有统计学意义（P＜0.05），Bax的阳性率分别为50.00%vs.44.10%，差异无统计学意义（P＞0.05）；在癌组织中，随着肿瘤分化程度的降低（高、中、低），TCF21与Bax的阳性表达率依次降低（53.33%vs.28.57%vs.12.50%，P＜0.05）、（80.00%vs.38.10%vs.25.00%，P＜0.05)。TCF21、Bax在有淋巴结转移和无淋巴结转移的阳性率为21.21% vs.72.73%（P＞0.05）、36.36%vs.90.91%（P＜0.05），无淋巴结转移的癌组织中TCF21、Bax阳性率较高。TCF21、Bax在TNM分期I+II期和III期阳性率为44.12%vs.00.00%（P＜0.05）、29.41%vs.40.00%（P＞0.05)。结论 TCF21在癌组织中表达下调与Bax呈正相关，与肿瘤的分化程度、淋巴结转移以及TNM分期有关，可能提示肿瘤恶性程度及预后。
목적：탐토억암기인TCF21급조망상관기인Bax재비소세포폐암（NSCLC）중적표체급림상의의。방법수집엄서빈양현인민의원2013～2015년흉외과절제적NSCLC표본공44례（함암급암방）。채용면역조직화학방법（ABC법）검측TCF21、Bax재폐암조직급정상조직중적표체，분석NSCLC조직중TCF21、Bax적표체급여림상병리특정적관계。결과재폐암조직급암방조직중，TCF21적양성솔위34.09%vs.56.81%，차이유통계학의의（P＜0.05），Bax적양성솔분별위50.00%vs.44.10%，차이무통계학의의（P＞0.05）；재암조직중，수착종류분화정도적강저（고、중、저），TCF21여Bax적양성표체솔의차강저（53.33%vs.28.57%vs.12.50%，P＜0.05）、（80.00%vs.38.10%vs.25.00%，P＜0.05)。TCF21、Bax재유림파결전이화무림파결전이적양성솔위21.21% vs.72.73%（P＞0.05）、36.36%vs.90.91%（P＜0.05），무림파결전이적암조직중TCF21、Bax양성솔교고。TCF21、Bax재TNM분기I+II기화III기양성솔위44.12%vs.00.00%（P＜0.05）、29.41%vs.40.00%（P＞0.05)。결론 TCF21재암조직중표체하조여Bax정정상관，여종류적분화정도、림파결전이이급TNM분기유관，가능제시종류악성정도급예후。
Objective To investigate the tumor suppressor gene TCF21 and apoptosis related gene Bax expression and clinical significance in non-small cell lung cancer (NSCLC).Methods Peoples’Hospital of Binyang County in Guangxi from the year of 2013 to 2015 surgical resection of NSCLC specimens of 44 patients (including adjacent and cancer). Immunohisto chemical method (ABC method) to detect TCF21, Bax expression in lung cancer tissues and normal tissues, analyze the relationship between NSCLC tissues TCF21, Bax and its relationship with clinicopathological features.Results In lung cancer and adjacent tissues, TCF21 positive rate of 34.09%vs. 56.81%, the difference was statistically significant (P<0.05), the positive rate of Bax were 50.00%vs. 44.10% difference was not statistically significant (P>0.05), in cancer tissue, with lower degree of tumor differentiation (high, medium, low), TCF21 and positive expression of Bax successively lower (53.33%vs. 28.57%vs. 12.50%,P<0.05 ), (80.00%vs. 38.10%vs. 25.00%,P <0.05). TCF21, Bax in turn and no lymph node metastasis, the positive rate 21.21%vs. 72.73% (P>0.05), 36.36%vs. 90.91% (P<0.05), without lymph node metastasis of carcinoma TCF21, Bax positive rate is higher. TCF21, Bax in TNM stage I+II and III positive rate was 44.12%vs. 00.00% (P<0.05), 29.41%vs. 40.00% (P>0.05).Conclusion TCF21 expression in cancer tissues down and Bax was positively correlated with the degree of tumor differentiation, lymph node metastasis and TNM staging, it might prompt tumor malignancy and prognosis.
