目的:探讨高通量血液透析对老年维持性血液透析患者微炎症状态的影响。方法选取40例老年维持性血液透析患者,入选前均使用低通量透析器,进入研究阶段后使用15UC或FX60聚砜膜高通量透析器,透析液流量为500 ml/min,血流量范围在200~300 ml/min,透析时间每次4 h,每周2~3次。于研究开始前、治疗6个月、治疗12个月分别采透析前清晨空腹肘正中静脉血标本检测超敏C反应蛋白(h-CRP)、白细胞介素6(IL-6)、肿瘤坏死因子-α(TNF-α)的水平,观察其变化情况。结果治疗6个月、治疗12个月与研究开始前相比,患者h-CRP、IL-6、TNF-α水平明显下降(P<0.01)。结论高通量血液透析对老年维持性血液透析患者微炎症状态有明显改善作用。

목적:탐토고통량혈액투석대노년유지성혈액투석환자미염증상태적영향。방법선취40례노년유지성혈액투석환자,입선전균사용저통량투석기,진입연구계단후사용15UC혹FX60취풍막고통량투석기,투석액류량위500 ml/min,혈류량범위재200~300 ml/min,투석시간매차4 h,매주2~3차。우연구개시전、치료6개월、치료12개월분별채투석전청신공복주정중정맥혈표본검측초민C반응단백(h-CRP)、백세포개소6(IL-6)、종류배사인자-α(TNF-α)적수평,관찰기변화정황。결과치료6개월、치료12개월여연구개시전상비,환자h-CRP、IL-6、TNF-α수평명현하강(P<0.01)。결론고통량혈액투석대노년유지성혈액투석환자미염증상태유명현개선작용。
Objective To study the effect on the senium patients of hemodialysis treated with high-flux hemodialysis.Methods 40 senium patients of hemodialysis which treated by low-flux hemodialysis before were treated by 15UC or FX60 polysulfone membrane high-flux hemodialysis during research stage. The dialysate lfow rate was 500 ml/min, blood lfow was 200~300 ml/min, dialytic time was 4 hours, 2~3 times one week. All the patients’ venous blood were drawn with an empty belly before research, 6 months after treatment and 12 months after treatment. The h-CRP, IL-6, TNF-α were detected to study the change. Results The h-CRP, IL-6, TNF-α after 6 months and 12 months treatment were significantly decreased than before (P<0.01).Conclusion High-lfux hemodialysis could improve the microinlfammatory state of senium patients with hemodialysis obviously.
