目的:探讨尿微量清蛋白及血清胱抑素C(CysC)联合检测对高尿酸血症合并糖尿病患者肾损伤的影响。方法选择我院300例高尿酸血症合并糖尿病患者,根据尿蛋白定性结果分为尿蛋白阳性组和尿蛋白阴性组,尿蛋白阳性组患者150例,尿蛋白阴性组患者150例,两组患者同时检测尿微量清蛋白(UMA)、CysC、血肌酐(Scr)及尿素氮(BUN)水平,并观察各项指标阳性率。结果两组患者UMA、CysC阳性率及2项指标联合检测阳性率均高于Scr、BUN阳性率,差异均有统计学意义(P<0.05),尿蛋白阳性组中,UMA、CysC阳性率及2项指标联合检测阳性率均高于尿蛋白阴性组对应各指标,差异有统计学意义(P<0.05),在尿蛋白阴性组中,UMA阳性率高于CysC阳性率,差异有统计学意义(P<0.05),2项指标联合检测阳性率均高于UMA、CysC阳性率,差异有统计学意义(P<0.05)。结论UMA、CysC联合检测高尿酸血症合并糖尿病患者可提高诊断肾损伤的敏感性。

목적:탐토뇨미량청단백급혈청광억소C(CysC)연합검측대고뇨산혈증합병당뇨병환자신손상적영향。방법선택아원300례고뇨산혈증합병당뇨병환자,근거뇨단백정성결과분위뇨단백양성조화뇨단백음성조,뇨단백양성조환자150례,뇨단백음성조환자150례,량조환자동시검측뇨미량청단백(UMA)、CysC、혈기항(Scr)급뇨소담(BUN)수평,병관찰각항지표양성솔。결과량조환자UMA、CysC양성솔급2항지표연합검측양성솔균고우Scr、BUN양성솔,차이균유통계학의의(P<0.05),뇨단백양성조중,UMA、CysC양성솔급2항지표연합검측양성솔균고우뇨단백음성조대응각지표,차이유통계학의의(P<0.05),재뇨단백음성조중,UMA양성솔고우CysC양성솔,차이유통계학의의(P<0.05),2항지표연합검측양성솔균고우UMA、CysC양성솔,차이유통계학의의(P<0.05)。결론UMA、CysC연합검측고뇨산혈증합병당뇨병환자가제고진단신손상적민감성。
Objective To study the effects of combined detection of urine microalbumin and serum cystatin C on renal injury of patients with hyperuricemia combined with diabetes mellitus.Methods 300 cases of patients with hyperuricemia combined with diabetes mellitus were divided into the urine protein positive group and urine protein negative group according to the basis of qualitative urine protein, In the urine protein positive group 150 patients, 150 patients with proteinuria negative group, simultaneous detection of urinary albumin (UMA), serum cystatin C CysC, serum creatinine (Scr), biood urea nitrogen (BUN) of two groups of patients, the positive rate of the above index was observed.Results The positive rate of CysC and UMA and the combined detection in the two patients was higher than the positive rate of Scr, BUN, there was signiifcant (P<0.05), the positive rate of CysC and UMA and the combined detection in the urine protein positive group was significantly higher than that of urine protein negative group, the difference was statistically significant (P<0.05), the positive rate of UMA in the urine protein negative group was higher than that of the positive rate of CysC, the difference was statistically signiifcant (P<0.05), the positive rate of combined detection were higher than that of UMA, CysC, the difference was statistically signiifcant (P<0.05). ConclusionThe combined detection of urine microalbumin and serum CysC can improve the sensitivity of diagnosis of renal injury in patients with hyperuricemia combined with diabetes.
