目的探讨糖尿病周围神经病变(DPN)的相关危险因素.方法选取2012年1月至2015年1月至我院就诊的114例2型糖尿病(T2DM)患者,根据有无周围神经病变分为DPN组和非糖尿病周围神经病组(NDPN组).检测患者空腹血糖(FPG)、餐后2小时血糖(2hPG)、糖化血红蛋白(HbA1c)、血脂及同型半胱氨酸(Hcy).结果 2组患者性别比、BMI、血脂、血压及血清肌酐均无统计学差异(P>0.05);两组年龄、病程、HbA1c、FPG、2hPG、Hcy差异有统计学意义(P<0.05).Logistic分析显示,糖尿病病程、HbA1c、Hcy是DPN的独立影响因素.结论 2型糖尿病患者的病程、HbA1c、Hcy是DPN的相关危险因素;对于年龄大、病程长、HbA1c及Hcy高的患者,临床医生应早期对DPN和相关危险因素进行预防和干预.
목적 탐토당뇨병주위신경병변(DPN)적상관위험인소.방법 선취2012년1월지2015년1월지아원취진적114례2형당뇨병(T2DM)환자,근거유무주위신경병변분위DPN조화비당뇨병주위신경병조(NDPN조).검측환자공복혈당(FPG)、찬후2소시혈당(2hPG)、당화혈홍단백(HbA1c)、혈지급동형반광안산(Hcy).결과 2조환자성별비、BMI、혈지、혈압급혈청기항균무통계학차이(P>0.05);량조년령、병정、HbA1c、FPG、2hPG、Hcy차이유통계학의의(P<0.05).Logistic분석현시,당뇨병병정、HbA1c、Hcy시DPN적독립영향인소.결론 2형당뇨병환자적병정、HbA1c、Hcy시DPN적상관위험인소;대우년령대、병정장、HbA1c급Hcy고적환자,림상의생응조기대DPN화상관위험인소진행예방화간예.
Objective To investigate the risk factors in patients with diabetic peripheral neuropathy.Methods From January 2012 to January 2015,114 patients with T2DM treated at our hospital were chosen and were divided into a DPN group and an NDPN group according to whether complicated with peripheral neuropathy.Fasting plasma glucose(FPG), 2h postprandial glucose(2hPG), glycosylated hemoglobin A1c(HbA1c), blood lipid, and homocysteine (Hcy) were detected.Results There were no statistical differences in gender, BMI, blood lipid, SBP, DBP, and Scr between these two groups (P>0.05).There were statistical differences in age, the duration of diabetes, HbA1c, FPG, 2hPG.and Hcy between these two groups(P<0.05).Logistic analysis showed that the duration of diabetes, HbAlc, and Hcy were independent risk factors for DPN.Conclusions The duration of diabetes, HbAlc, and Hcy are associated risk factors for DPN.And old age, long duration of diabetes, and high HbA1c and Hcy increase the risk of the occurrence of DPN.Clinicians should prompt prevention and intervention in early stage of diabetes and related risk factors for DPN.
