目的：探讨ERCP技术联合腹腔镜胆囊切除术(LC)在胆囊结石并胆管结石治疗中的临床应用。方法：收治胆囊结石并胆管结石患者25例，先用ERCP技术取出胆管结石，放置鼻胆引流管或塑料支架引流；术后1～7 d，患者无腹痛、发热，白细胞计数正常，再行LC。结果：22例(88%)成功完成ERCP取石术，21例置鼻胆管，1例置塑料支架，2例仅造影诊断，1例因解剖变异插管失败。22例患者完成LC手术，1例患者行开腹手术，1例患者因心肺功能差无法耐受手术行塑料支架植入术。无ERCP技术和LC手术相关并发症。结论：ERCP技术联合LC对胆囊结石合并胆总管结石的治疗，减轻患者的痛苦，缩短住院时间，是外科微创术式的可靠、可行的选择。
목적：탐토ERCP기술연합복강경담낭절제술(LC)재담낭결석병담관결석치료중적림상응용。방법：수치담낭결석병담관결석환자25례，선용ERCP기술취출담관결석，방치비담인류관혹소료지가인류；술후1～7 d，환자무복통、발열，백세포계수정상，재행LC。결과：22례(88%)성공완성ERCP취석술，21례치비담관，1례치소료지가，2례부조영진단，1례인해부변이삽관실패。22례환자완성LC수술，1례환자행개복수술，1례환자인심폐공능차무법내수수술행소료지가식입술。무ERCP기술화LC수술상관병발증。결론：ERCP기술연합LC대담낭결석합병담총관결석적치료，감경환자적통고，축단주원시간，시외과미창술식적가고、가행적선택。
Objective:To explore the clinical application of ERCP combined with laparoscope for treatment of gallstone with common bile duct stones.Methods:25 patients with gallstone and common bile duct stones were selected,and bile duct stones were removed by using ERCP technique,and then,the patients were given nasal biliary drainage tube or plastic stent drainage.At 1 to 7 days after surgery,patients had no abdominal pain and fever,white blood cell count was normal,and then,they were given LC. Results:ERCP stone removal in 22 cases(88%) were successful,21 cases were given nasal bile duct,1 case was given plastic stents, 2 cases were diagnosed by radiography,1 case of intubation was failed due to anatomical variation.22 patients underwent LC operation,1 case underwent open surgery,1 patient underwent plastic stent implantation because he could not tolerate surgery due to poor heart and lung function.There was no ERCP technique and LC operation related complications.Conclusion:ERCP combined with laparoscope for treatment of gallstone with common bile duct stones can reduce the patient's pain,shorten the length of hospital stay,and it was a reliable and feasible choice for minimally invasive surgery.
