目的：体会睾丸定向挤捏穿刺术与开放式睾丸活检的取精效果。方法：收治睾丸活检患者639例，其中开放式睾丸活检302例，睾丸定向挤捏穿刺术337例，比较两组手术取精成功率、术后并发症、手术难易程度等。结果：开放式睾丸活检取精成功率100%，术后第2天睾丸疼痛率19.2%，术后阴囊血肿5例，感染0，术后3个月随访性功能障碍2例。睾丸定向挤捏穿刺取精术成功率98.2%，术后睾丸疼痛发生率4.4%，术后阴囊血肿1例，术后感染0，术后性功能障碍1例。结论：定向挤捏睾丸取精操作简单，损伤小，患者心理容易接受，并能够提高操作者的工作效率。
목적：체회고환정향제날천자술여개방식고환활검적취정효과。방법：수치고환활검환자639례，기중개방식고환활검302례，고환정향제날천자술337례，비교량조수술취정성공솔、술후병발증、수술난역정도등。결과：개방식고환활검취정성공솔100%，술후제2천고환동통솔19.2%，술후음낭혈종5례，감염0，술후3개월수방성공능장애2례。고환정향제날천자취정술성공솔98.2%，술후고환동통발생솔4.4%，술후음낭혈종1례，술후감염0，술후성공능장애1례。결론：정향제날고환취정조작간단，손상소，환자심리용역접수，병능구제고조작자적공작효솔。
Objective:To experience the sperm extraction effects of testicular directional squeeze centesis and open testicular biopsy.Methods:639 patients with testicular biopsy were selected.302 cases were open testicular biopsy.337 cases were testicular directional squeeze centesis.The success rates of operation sperm extraction,postoperative complications and operation difficult degrees of two groups were compared.Results:The success rate of sperm extraction of open testicular biopsy was 100% ,the testicular pain rate on the second day post operative was 19.2%,the postoperative scrotum hematoma had 5 cases,the infection had 0 cases,the sexual dysfunction after postoperative 3 months follow-up had 2 cases.The success rates of testicular directional squeeze centesis sperm extraction operation was 98.2%,the postoperative testicular pain rate was 4.4%,the postoperative scrotum hematoma had 1 case,the postoperative infection had 0 cases,the postoperative sexual dysfunction had 1 case.Conclusion:The directional squeeze testicular sperm extraction operation is simple,has small damage,the patients' psychological is easy to accept, and it can improve the work efficiency of the operator.
