目的:探究结构脂肪乳与中/长链脂肪乳支持治疗对老年直肠癌患者术后蛋白质和脂质代谢及免疫功能的影响,为临床选择提供依据。方法选取限期手术的老年直肠癌患者79例,根据术后肠外营养支持脂肪乳剂配方不同分为两组,对照组41例选用20%中/长链脂肪乳剂治疗,研究组38例选用20%结构脂肪乳注射液治疗,于支持前、支持3天后、支持7天后分别采集外周血,监测患者免疫功能、蛋白质和脂质代谢等指标变化,同时记录营养支持过程中出现的并发症状况。结果两组蛋白质指标均较支持前提高(P<0.05),研究组除支持前外,其他各时间点均高于对照组(P<0.05);两组血脂指标较支持前有所改善(P<0.05),研究组除支持前外,其他各时间点均低于对照组(P<0.05);两组间CD3+、CD4+、CD4+/CD8+水平均较支持前提高(P<0.05),研究组除支持前外,其他各时间点均高于对照组(P<0.05);对照组中有2例患者输注营养液时出现皮肤潮红、恶心等不适,研究组患者无明显不适,两组患者无严重并发症发生。结论结构脂肪乳肠外营养支持可以有效改善老年直肠癌患者术后蛋白质和脂质代谢,促进患者术后免疫功能恢复。

목적:탐구결구지방유여중/장련지방유지지치료대노년직장암환자술후단백질화지질대사급면역공능적영향,위림상선택제공의거。방법선취한기수술적노년직장암환자79례,근거술후장외영양지지지방유제배방불동분위량조,대조조41례선용20%중/장련지방유제치료,연구조38례선용20%결구지방유주사액치료,우지지전、지지3천후、지지7천후분별채집외주혈,감측환자면역공능、단백질화지질대사등지표변화,동시기록영양지지과정중출현적병발증상황。결과량조단백질지표균교지지전제고(P<0.05),연구조제지지전외,기타각시간점균고우대조조(P<0.05);량조혈지지표교지지전유소개선(P<0.05),연구조제지지전외,기타각시간점균저우대조조(P<0.05);량조간CD3+、CD4+、CD4+/CD8+수평균교지지전제고(P<0.05),연구조제지지전외,기타각시간점균고우대조조(P<0.05);대조조중유2례환자수주영양액시출현피부조홍、악심등불괄,연구조환자무명현불괄,량조환자무엄중병발증발생。결론결구지방유장외영양지지가이유효개선노년직장암환자술후단백질화지질대사,촉진환자술후면역공능회복。
Objective To investigate the effect of structural fat emulsion and medium/ long chain fat emulsion on the protein and lipid metabolism and immune function in elderly patients with rectal cancer, providing knowledge and alternatives for clinical options. Method A total of 79 cases of elderly rectal cancer patients that were adminis-tered with time-limited operation were included in this study, and were divided into two groups based on formula-tions of fat emulsion delivered for postoperative parenteral nutrition support, in which the control group (41 cases) was applied with 20% medium/ long chain fat emulsion, and the study group (38 cases) was treated with 20% struc-tural fat emulsion, and peripheral blood were collected from all patients before injection, 3 days and 7 days after in-jection to monitor immune function, protein and lipid metabolism changes. Complications occurred in the period were also documented. Result The protein index of both groups were improved after injection (P < 0.05), and all values of the study group at each time point were higher than that of the control group, except for those before injection (P < 0.05); The lipids profile of both groups were improved after injection (P < 0.05), and all values of the study group at each time point were lower than that of the control group (P < 0.05), except for those before injection; The CD3 +, CD4 +, CD4 +/ CD8 + levels of both groups were lower after injection (P < 0.05), and all values of the study group at each time point were lower than that of the control group (P< 0.05), except for those before injection;And there were 2 patients in the control group that developed flushing, nausea and discomfort during injection, while the study group had no adverse reactions, and no severe complications were observed in both groups. Conclusion The structural fat emulsion not only improves the protein and lipid metabolism in elderly patients with rectal cancer, but also promotes the recovery of postoperative immune function.
