目的：探讨小骨窗开颅手术与微创穿刺手术治疗高血压脑出血的临床效果。方法：收治高血压脑出血患者60例，随机分为对照组与观察组。对照组采用小骨窗开颅手术治疗，观察组开展微创穿刺治疗，比较两组临床效果。结果：两组术后血肿清除率、再出血率、并发症发生率与死亡率比较，差异均无统计学意义(P＞0.05)。观察组手术时间、术后导管拔除时间与住院时间均显著短于对照组，差异具有统计学意义(P＜0.05)。结论：微创穿刺手术治疗高血压脑出血的预后效果同开颅手术基本相当，但该种治疗方式缩短了手术时间，降低了治疗的经济成本。
목적：탐토소골창개로수술여미창천자수술치료고혈압뇌출혈적림상효과。방법：수치고혈압뇌출혈환자60례，수궤분위대조조여관찰조。대조조채용소골창개로수술치료，관찰조개전미창천자치료，비교량조림상효과。결과：량조술후혈종청제솔、재출혈솔、병발증발생솔여사망솔비교，차이균무통계학의의(P＞0.05)。관찰조수술시간、술후도관발제시간여주원시간균현저단우대조조，차이구유통계학의의(P＜0.05)。결론：미창천자수술치료고혈압뇌출혈적예후효과동개로수술기본상당，단해충치료방식축단료수술시간，강저료치료적경제성본。
Objective:To explore the clinical effect of craniotomy with small bone hole and minimally puncture surgery treating hypertensive cerebral hemorrhage.Methods:60 cases of patients with hypertensive cerebral hemorrhage were randomly divided into the control group and the observation group.The control group were treated with craniotomy with small bone hole,the observation group were given minimally puncture surgery.The clinical effect of the two groups were compared.Results:There were no statistically significant differences of postoperative hematoma clearance rate,rebleeding rate,incidence of complications and mortality between the two groups(P>0.05).The operative time,postoperative catheter removal time and hospital stay of the observation group were significantly shorter than that of the control group,the differences had statistical significance(P<0.05). Conclusion:Minimally puncture surgery in the treatment of hypertensive cerebral hemorrhage had the basic same prognosis effect as the craniotomy,but this kind of treatment shortened the operation time and reduced the economic costs.
