目的：探讨外伤性脑梗死超早期诊断后的治疗策略。方法对收治的各类颅脑外伤患者中怀疑有外伤性脑梗死119例患者进行了早期（磁共振弥散加权成像） DWI检查，发现30例并发外伤性脑梗死。分为：回顾组12例在常规治疗基础上加用依达拉奉治疗，观察组18例在常规治疗基础上加用依达拉奉及神经节苷脂治疗，治疗后30 d，用格拉斯哥预后评分（ GOS）的标准，评定疗效。结果回顾组12例中基本治愈5例，显效3例，有效1例，无效3例，有效率75.0％。观察组18例中基本治愈7例，显效4例，有效5例，无效2例，有效率88.9％。2组比较统计学差异有显著性（ P＜0.05）。结论颅脑损伤后应早期发现脑梗死，在常规治疗基础上采取有自由基清除剂及神经保护剂，能降低患者的致残率。
目的：探討外傷性腦梗死超早期診斷後的治療策略。方法對收治的各類顱腦外傷患者中懷疑有外傷性腦梗死119例患者進行瞭早期（磁共振瀰散加權成像） DWI檢查，髮現30例併髮外傷性腦梗死。分為：迴顧組12例在常規治療基礎上加用依達拉奉治療，觀察組18例在常規治療基礎上加用依達拉奉及神經節苷脂治療，治療後30 d，用格拉斯哥預後評分（ GOS）的標準，評定療效。結果迴顧組12例中基本治愈5例，顯效3例，有效1例，無效3例，有效率75.0％。觀察組18例中基本治愈7例，顯效4例，有效5例，無效2例，有效率88.9％。2組比較統計學差異有顯著性（ P＜0.05）。結論顱腦損傷後應早期髮現腦梗死，在常規治療基礎上採取有自由基清除劑及神經保護劑，能降低患者的緻殘率。
목적：탐토외상성뇌경사초조기진단후적치료책략。방법대수치적각류로뇌외상환자중부의유외상성뇌경사119례환자진행료조기（자공진미산가권성상） DWI검사，발현30례병발외상성뇌경사。분위：회고조12례재상규치료기출상가용의체랍봉치료，관찰조18례재상규치료기출상가용의체랍봉급신경절감지치료，치료후30 d，용격랍사가예후평분（ GOS）적표준，평정료효。결과회고조12례중기본치유5례，현효3례，유효1례，무효3례，유효솔75.0％。관찰조18례중기본치유7례，현효4례，유효5례，무효2례，유효솔88.9％。2조비교통계학차이유현저성（ P＜0.05）。결론로뇌손상후응조기발현뇌경사，재상규치료기출상채취유자유기청제제급신경보호제，능강저환자적치잔솔。
Objective To investigate the treatment strategy of traumatic cerebral infarction after ultra early diagnosis. Methods 119 cases of craniocerebral trauma patients underwent early diffusion-weighted imaging( DWI) examination,and 30 were identified to be complicated with traumatic cerebral infarction.Among the 30 cases,12 in the review group were treated in the conventional way in addition to edaravone treatment;18 cases in the observation group were treated in the conventional way combined with edaravone and ganglioside treatment.30 days afterwards,prognosis was evaluated with Glasgow score(GOS) standard.Results As for the review group,5 cases were basically cured,3 cases markedly effective,1 case effective,and 3 ca-ses ineffective,with a total effective rate of 75.0%.In the observation group,on the other hand,7 cases were basically cured, 4 cases markedly effective,5 cases effective,and 2 cases ineffective,with a total effective rate of 88.9%.The two groups had statistically significant difference(P<0.05).Conclusion Cerebral infarction after craniocerebral injury should be detected early.On the basis of conventional therapy,application of free radical scavenger and neuroprotective agent can reduce the disa-bility rate of the treatment.
