目的：探讨胶原酶治疗椎间盘膨出的临床价值。方法对61例椎间盘膨出症患者在CT定位下经皮侧方进针直接进入椎间盘内部，并注入胶原酶。结果61例穿刺65针，穿刺成功率100%，全部患者短期(3-6个月)内腰部疼痛及肢体麻木症状明显减轻,28例于术后3-6个月复查CT，均可见椎间盘内出现空腔，70%椎间盘膨出度无明显改变，20%椎间盘膨出度轻微回缩,10%神经根压迫症状有改善。结论 CT定位下穿刺治疗椎间盘膨出，操作方便，安全性高，创伤小。
목적：탐토효원매치료추간반팽출적림상개치。방법대61례추간반팽출증환자재CT정위하경피측방진침직접진입추간반내부，병주입효원매。결과61례천자65침，천자성공솔100%，전부환자단기(3-6개월)내요부동통급지체마목증상명현감경,28례우술후3-6개월복사CT，균가견추간반내출현공강，70%추간반팽출도무명현개변，20%추간반팽출도경미회축,10%신경근압박증상유개선。결론 CT정위하천자치료추간반팽출，조작방편，안전성고，창상소。
Objective To investigate the clinical value of collagenase in the treatment of lumbar disk bulging.Methods 61 cases of lumbar disc bulging patients accepted CT-guided percutaneous injection of collagenase chemonucleolysis.Results A total of 65 needles were punctured, puncture success rate of 100%. The lumbar pain and numbness of limb were relieved after surgery in short-term (3-6 months), and intervertebral disc cavity could be seen in 28 cases of CT review after surgery. 70% patients had no signiifcant change in the degree of disc bulge, 20% of the mild disc bulge retraction, 10% of the nerve root compression symptoms have improved.Conclusion It’s convenient, safe, and less trauma by CT-guided percutaneous injection of collagenase chemonucleolysis in the treatment of lumbar disc bulging.
