目的：比较甲强龙与氢化可的松治疗慢性阻塞性肺病（COPD）急性加重期的临床疗效。方法选择2013年1—6月入住湖北省新华医院的 COPD 急性加重期患者62例，随机分为甲强龙组30例和氢化可的松组32例。甲强龙组给予甲强龙治疗，氢化可的松组给予氢化可的松治疗，比较两组的肺功能〔第1秒用力呼气容量（FEV1）、肺活量（FVC）〕和动脉血气分析指标。结果治疗后第3天两组 FEV1、FVC、PaO2、PaCO2比较，差异无统计学意义（P >0.05），第10天两组 FEV1、FVC、PaO2、PaCO2比较，差异有统计学意义（P <0.05）。结论甲强龙与氢化可的松治疗 COPD 急性加重期的疗效显著，甲强龙改善肺功能的效果优于氢化可的松。
목적：비교갑강룡여경화가적송치료만성조새성폐병（COPD）급성가중기적림상료효。방법선택2013년1—6월입주호북성신화의원적 COPD 급성가중기환자62례，수궤분위갑강룡조30례화경화가적송조32례。갑강룡조급여갑강룡치료，경화가적송조급여경화가적송치료，비교량조적폐공능〔제1초용력호기용량（FEV1）、폐활량（FVC）〕화동맥혈기분석지표。결과치료후제3천량조 FEV1、FVC、PaO2、PaCO2비교，차이무통계학의의（P >0.05），제10천량조 FEV1、FVC、PaO2、PaCO2비교，차이유통계학의의（P <0.05）。결론갑강룡여경화가적송치료 COPD 급성가중기적료효현저，갑강룡개선폐공능적효과우우경화가적송。
Objective To compare the clinical effect of methylprednisolone and hydrocortisone in the treatment of a-cute exacerbation of chronic obstructive pulmonary disease(COPD). Methods A total of 62 patients with acute exacerbation of COPD hospitalized from January 2013 to June 2013 in Hubei Xinhua Hospital were selected,and they were randomly divided into 30 cases of methylprednisolone group and 32 cases of hydrocortisone group. Methylprednisolone group was given methylpred-nisolone therapy,hydrocortisone group was given hydrocortisone therapy,lung function(FEV1、FVC)and arterial blood gas a-nalysis between the two groups were compared. Results 3 days after treatment FEV1 ,FVC,PaO2 ,PaCO2 between the two groups were compared,the differences were not statistically significant( P > 0. 05),10 days after treatment FEV1 ,FVC, PaO2 ,PaCO2 between the two groups were compared,the differences were statistically significant(P < 0. 05). Conclusion Methylprednisolone and hydrocortisone both have remarkable effect in treatment of COPD patients with acute exacerbation,the lung function improving of methylprednisolon is better than hydrocortisone.
