目的:探讨低糖高脂肪营养支持治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者的临床疗效。方法选取2013年1月—2015年1月梧州市红十字会医院收治的 COPD 合并呼吸衰竭患者72例,按照随机数字表法分为对照组与试验组,各36例。对照组患者予以常规营养支持治疗,试验组患者予以低糖高脂肪营养支持治疗。观察两组患者治疗前后呼吸功能(呼吸商、每分钟通气量、耗氧量)、pH 值、PCO2、PO2指标情况。结果治疗前两组患者呼吸商、每分钟通气量、耗氧量比较,差异无统计学意义( P >0.05);治疗后试验组患者呼吸商、每分钟通气量、耗氧量低于对照组,pH 值、PCO2低于对照组,PO2高于对照组,差异有统计学意义(P <0.05)。结论低糖高脂肪营养支持治疗 COPD 合并呼吸衰竭患者的临床疗效显著,可改善患者肺功能及相关指标。

목적:탐토저당고지방영양지지치료만성조새성폐질병(COPD)합병호흡쇠갈환자적림상료효。방법선취2013년1월—2015년1월오주시홍십자회의원수치적 COPD 합병호흡쇠갈환자72례,안조수궤수자표법분위대조조여시험조,각36례。대조조환자여이상규영양지지치료,시험조환자여이저당고지방영양지지치료。관찰량조환자치료전후호흡공능(호흡상、매분종통기량、모양량)、pH 치、PCO2、PO2지표정황。결과치료전량조환자호흡상、매분종통기량、모양량비교,차이무통계학의의( P >0.05);치료후시험조환자호흡상、매분종통기량、모양량저우대조조,pH 치、PCO2저우대조조,PO2고우대조조,차이유통계학의의(P <0.05)。결론저당고지방영양지지치료 COPD 합병호흡쇠갈환자적림상료효현저,가개선환자폐공능급상관지표。
Objective To explore the effect of low sugar and high fat nutrition support in treating chronic obstructive pulmonary disease(COPD)and respiratory failure. Methods A total of 72 patients with COPD and respiratory failure were se-lected in Red Cross Hospital of Wuzhou from January 2013 to January 2015,according to the stochastic number table method, they were divided into control group and test group,36 cases in each group. Control group were given conventional nutritional support treatment,test group were given low sugar and high fat nutrition support treatment. Before and after treatment,respirato-ry function(respiratory quotient,minute ventilation volume,oxygen consumption),pH,PCO2 ,PO2 between the two groups were compared. Results Before treatment,respiratory quotient,minute ventilation volume,oxygen consumption showed no significant differences between the two groups(P > 0. 05),after treatment,respiratory quotient,minute ventilation volume, oxygen consumption of test group were lower than those of control group,pH,PCO2 of test group were lower than those of control group,PO2 of test group was higher than that of control group(P < 0. 05). Conclusion Low sugar and high fat nutrition sup-port has notable curative effect in the treatment of COPD and respiratory failure,can improve lung function of patients with and related biochemical indicators.
