目的 分析2014年首都医科大学附属北京中医医院产β-内酰胺酶金黄色葡萄球菌的分布特点及耐药性. 方法 收集送检标本15 620例,对产β-内酰胺酶金黄色葡萄球菌进行分离,观察其临床分布特征及耐药性. 结果 共分离非重复病原菌5075株,其中金黄色葡萄球菌526株(10. 4%),包括产β-内酰胺酶金黄色葡萄球菌301株(57. 2%),其来源主要为创口分泌液和痰液,分布科室主要有重症监护病房、皮肤科和疮疡外科,对青霉素、红霉素、环丙沙星和苯唑西林等耐药率较高,对呋喃妥因、利奈唑胺和万古霉素的耐药率均为0;多引起呼吸机相关感染、下呼吸道感染和皮肤化脓性感染等. 结论 产β-内酰胺酶金黄色葡萄球菌分布广泛,其对多种抗生素耐药率高,临床应高度重视.

Objective To analyze the characteristics of distribution and drug resistance of staphylococcus aureus producingβ-lactamase in Beijing TCM Hospital affiliated to the Capital Medical University in 2014. Methods A total of 15 620 samples were collected, and the staphylococcus aureus producingβ-lactamase were isolated, and then the charac-teristics of clinical distribution and drug resistance were observed. Results A total of 5075 bacterium strains were isola-ted, including 526 strains ( 10. 4%) of staphylococcus aureus, 301 strains ( 57. 2%) of staphylococcus aureus producingβ-lactamase. The staphylococcus aureus producingβ-lactamase were mainly from wound secretion and sputum, and were mainly distributed in ICU, dermatology department and department of sores ulceration surgery. The staphylococ-cus aureus producing β-lactamase had a high rate of drug resistance against Penicillin, Erythromycin, Ciprofloxacin and Oxacillin, but had 0% drug resistance rate against Nitrofurantoin, Linezolid and Vancomycin. The staphylococcus aureus producing β-lactamase could probably cause ventilator associated infections, lower respiratory tract infections, purulent skin infections, etc. Conclusion Staphylococcus aureus producingβ-lactamase is widely distributed, and has high rates of drug resistance against many antibiotics. So clinicians should pay more attention to it.
