目的 探讨四君子汤联合精氨酸对胃癌术后营养状态及免疫功能的影响. 方法 2010年4月—2014年10月收治的晚期胃癌100例,随机分为观察组和对照组,每组50例,两组均术后早期给予肠内营养支持,观察组在此基础上加用四君子汤与精氨酸,观察两组术后营养指标及免疫功能情况. 结果 两组患者术后8d血红蛋白、总胆固醇、甘油三酯、低密度脂蛋白浓度较术前有所降低,且观察组上述指标明显高于对照组( P<0. 05 );两组术后8 d血清CD3、CD4、自然杀伤细胞(NK)、IgG、IgM、IgA浓度与CD4/CD8较术前降低,CD8较术前升高(P<0. 05);术后8 d观察组血清CD3、CD4、NK、CD4/CD8、IgG、IgM、IgA浓度高于对照组,CD8低于对照组(P<0. 05);两组术后8 d 白介素(IL)-2浓度较术前降低,且观察组高于对照组(P<0. 05);两组术后8 d IL-6浓度较术前升高,且观察组低于对照组(P<0. 05). 结论 肠内营养基础上给予四君子汤联合精氨酸可以进一步改善胃癌患者术后的营养状态与免疫功能,促进患者康复.

Objective To study the effect of Sijunzi decoction combined with Arginine ( Arg) on postoperative nutritional status and immune function of patients with advanced gastric carcinoma. Methods A total of 100 patients with advanced gastric carcinoma from April 2010 to October 2014 were randomly divided into observation group (n=50) and control group (n=50). All the patients received early enteral nutritional support, and then the observation group was added with Sijunzi decoction and Arg. The postoperative nutritional indicators and immune function were observed in the two groups. Results In the two groups, the levels of hemoglobin ( Hb) , total cholesterol ( TC) , triglyceride ( TG) , low density lipoprotein ( LDL) concentration on the postoperative 8 th d were significantly lower than those before the oper-ation, and the levels in the observation group were significantly higher than those in the control group (P<0. 05);on the postoperative 8th d, the concentrations of serum CD3, CD4, natural killer cell (NK), immunoglobulin G (IgG), IgM, IgA and CD4/CD8 value were lower, while the values of CD8 concentration were higher than those before the operation (P<0. 05). On the postoperative 8th d, the concentrations of serum CD3, CD4, NK, IgG, IgM, IgA and CD4/CD8 val-ue were higher, while values of the CD8 concentration were lower in the observation group than those in the control group (P<0. 05);the concentration of IL-2 was lower than preoperative levels(P<0. 05), the concentration values of inter-leukin-2 (IL-2) were lower than those before the operation in the two groups, and the value in the observation group was significantly higher than that in the control group (P<0. 05);the concentration values of IL-6 were higher than those be-fore the operation in the two groups, and the value in the observation group was significantly lower than that in the control group (P<0. 05). Conclusion The Sijunzi decoction combined with Arg based on enteral nutrition can further improve the nutritional status and immune function of postoperative patients with advanced gastric carcinoma, and can promote re-habilitation.
