目的：对某院住院患者用药合理性进行点评，为临床合理用药提供参考，保证患者用药安全有效。方法随机抽取某院2014年1月～12月住院病历360份进行用药点评，对不合理用药进行统计分析。结果360份病历中有87份病历存在用药不合理现象，主要归纳为无适应证用药、用法用量不适宜、给药途径不适宜三大类。结论我院住院患者临床药物治疗中存在着一些不合理现象。病历点评工作非常必要，药师应及时反馈点评信息，共同促进合理用药。
목적：대모원주원환자용약합이성진행점평，위림상합리용약제공삼고，보증환자용약안전유효。방법수궤추취모원2014년1월～12월주원병력360빈진행용약점평，대불합리용약진행통계분석。결과360빈병력중유87빈병력존재용약불합리현상，주요귀납위무괄응증용약、용법용량불괄의、급약도경불괄의삼대류。결론아원주원환자림상약물치료중존재착일사불합리현상。병력점평공작비상필요，약사응급시반궤점평신식，공동촉진합리용약。
Objective To evaluate the rationality of drug using in our hospital,and provide the reference for rational drug use in clinic. Ensure safty and efficacy in medication. Methods Choose 360 of the cases in 2014 in our hospital to comment and statistically analyze the irrational drug using. Results There are 87 cases exsit irrational drug using in total 360 cases. Conclusion It is necessary to carry out the medical record reviews work. Pharmacist should feedback the information to ensure the safty and efficient drug use.
