Breast ductal carcinoma in situ%Ultrasound%Digital mammography
目的 探讨高频超声联合数字乳腺X线摄影对乳腺导管内癌的诊断价值. 方法 回顾性分析56例经病理证实的乳腺导管内癌的影像学特征并比较两种检查方法的诊断准确率. 结果 超声表现为团块型24例,导管扩张型7例,单纯钙化型13例,数字乳腺X线摄影表现为钙化型21例,局部密度增高型12例,结构紊乱型9例,数字乳腺X线摄影对导管内癌诊断符合率为75 .0%( 42/56 ) ,高频超声对导管内癌诊断符合率为78.6%(44/56),诊断准确率比较,差异无统计学意义(P>0.05),高频超声联合数字乳腺X线摄影对导管内癌的诊断准确率为92.9%(52/56),显著高于超声诊断准确率(P<0.05). 结论 联合数字乳腺X线摄影及超声均对导管内癌有较高诊断准确率,但两者联合应用可提高诊断准确率.
目的 探討高頻超聲聯閤數字乳腺X線攝影對乳腺導管內癌的診斷價值. 方法 迴顧性分析56例經病理證實的乳腺導管內癌的影像學特徵併比較兩種檢查方法的診斷準確率. 結果 超聲錶現為糰塊型24例,導管擴張型7例,單純鈣化型13例,數字乳腺X線攝影錶現為鈣化型21例,跼部密度增高型12例,結構紊亂型9例,數字乳腺X線攝影對導管內癌診斷符閤率為75 .0%( 42/56 ) ,高頻超聲對導管內癌診斷符閤率為78.6%(44/56),診斷準確率比較,差異無統計學意義(P>0.05),高頻超聲聯閤數字乳腺X線攝影對導管內癌的診斷準確率為92.9%(52/56),顯著高于超聲診斷準確率(P<0.05). 結論 聯閤數字乳腺X線攝影及超聲均對導管內癌有較高診斷準確率,但兩者聯閤應用可提高診斷準確率.

Objective To estimate the accuracy of ultrasound combined with mammography in the diagnosis of ductal carcinoma in situ ( DCIS ) .Methods Imaging features of fifty-six patients with DCIS confirmed by pathology were retrospectively analyzed and estimated the accuracy .Results Mass type in 24,ductal ectasia type in 7,pure calcifi-cation type in 13 were observed by ultrasound .By mammography,calcification type in 21,local increased density in 12, architecture disorder type in 9.The coincidence rates of ultrasound and digital mammography of DCIS were 78.6%and 75.0%respectively.The accuracy was not statistically different (P>0.05).The coincidence rates of combination of both methods was 92.9%,higher than that of ultrasound (P<0.05).Conclusion Both ultrasound and digital mammography are useful diagnostic method of DCIS ,the diagnosic accuracy can be improved by combination of ultrasound and mam-mography .
