目的:探讨应用股薄肌肌腱重建内侧髌股韧带( MPFL)治疗复发性髌骨脱位的疗效。方法对14例复发性髌骨脱位患者采用自体股薄肌腱单束重建MPFL,同时常规行外侧支持带松解。影像学评价包括测量髌股适合角、外侧髌股角、髌骨外移率;应用Lysholm膝关节功能评分评价临床疗效。结果患者均获得随访,时间9~28个月。术后恐惧试验均阴性,随访期间无髌骨再脱位及骨折发生。末次随访时髌股适合角5.71°±1.27°、外侧髌股角3.44°±0.76°、髌骨外移率0.50±0.13、 Lysholm膝关节功能评分94.38分±2.67分,与术前比较差异均有统计学意义(P<0.01)。结论应用股薄肌肌腱单束法重建MPFL治疗复发性髌骨脱位近期效果可靠。

목적:탐토응용고박기기건중건내측빈고인대( MPFL)치료복발성빈골탈위적료효。방법대14례복발성빈골탈위환자채용자체고박기건단속중건MPFL,동시상규행외측지지대송해。영상학평개포괄측량빈고괄합각、외측빈고각、빈골외이솔;응용Lysholm슬관절공능평분평개림상료효。결과환자균획득수방,시간9~28개월。술후공구시험균음성,수방기간무빈골재탈위급골절발생。말차수방시빈고괄합각5.71°±1.27°、외측빈고각3.44°±0.76°、빈골외이솔0.50±0.13、 Lysholm슬관절공능평분94.38분±2.67분,여술전비교차이균유통계학의의(P<0.01)。결론응용고박기기건단속법중건MPFL치료복발성빈골탈위근기효과가고。
Objective To investigate the efficacy of arthroscopic single-bundle reconstruction of medial patellofemoral ligament( MPFL) with gracilis tendon autograft for patients with recurrent patellar dislocation. Methods 14 cases with recurrent dislocation of the patella were performed arthroscopic surgery. All patients were treated with single-bundle medial patellofemoral ligament reconstruction and lateral retinacular release. The imaging evaluation included congruence angle, lateral patellofemoral angle and lateral shift. The clinical therapeutic effect was evaluated with Ly-sholm scores. Results All patients were followed up for 9 ~28 months. The fear test was negative after operation. There was no redislocation and fracture of the patella during follow-up. The congruence angle,lateral patellofemoral angle and lateral shift were 5. 71° ± 1. 27°,3. 44° ± 0. 76° and 0. 50 ± 0. 13 respectively at the final follow-up. Ly-sholm score was 94. 38 ± 2. 67 at the final follow-up. It was showed statistically significant differences compared with preoperation ( P<0. 01 ) . Conclusions There is a reliable short-term effect of single-bundle medial patellofemoral ligament reconstruction with gracilis tendon autograft for recurrent patellar dislocation under arthroscopy.
