目的:探讨不同单发转移部位乳腺癌患者对循环肿瘤细胞( circulating tumor cell,CTC)检测的敏感性。方法采集119例乳腺癌单发转移(肝、肺、骨)患者的外周血7.5 mL,按转移部位分成3组,采用cellsearch方法检测血液标本中的CTC,统计分析各组检出率及阳性率,并比较其差异。结果晚期乳腺癌肝、肺、骨转移患者外周血中CTC的检出率分别为35.5%(11/31)、47.5%(19/40)、68.8%(33/48),骨转移组的检出率明显高于肺及肝转移组(P<0.05)。肝、肺、骨转移组CTC的阳性率分别为32.3%(10/31)、12.5%(5/40)、12.5%(6/48),肝转移组与肺及骨转移组比较差异有显著性意义(P<0.05)。结论 CTC检测对于晚期乳腺癌骨转移患者的敏感性较高。相比骨转移及肺转移,晚期乳腺癌肝转移患者的预后相对较差。

목적:탐토불동단발전이부위유선암환자대순배종류세포( circulating tumor cell,CTC)검측적민감성。방법채집119례유선암단발전이(간、폐、골)환자적외주혈7.5 mL,안전이부위분성3조,채용cellsearch방법검측혈액표본중적CTC,통계분석각조검출솔급양성솔,병비교기차이。결과만기유선암간、폐、골전이환자외주혈중CTC적검출솔분별위35.5%(11/31)、47.5%(19/40)、68.8%(33/48),골전이조적검출솔명현고우폐급간전이조(P<0.05)。간、폐、골전이조CTC적양성솔분별위32.3%(10/31)、12.5%(5/40)、12.5%(6/48),간전이조여폐급골전이조비교차이유현저성의의(P<0.05)。결론 CTC검측대우만기유선암골전이환자적민감성교고。상비골전이급폐전이,만기유선암간전이환자적예후상대교차。
Objective To evaluate the detection sensitivities and prognosis of advanced breast cancers with different metas-tases by detecting circulating tumor cells ( CTC) .Methods Blood samples (7.5 mL) were obtained from 119 patients of advanced breast cancer with different metastases ( liver, lung, bone) .Patients were divided into liver group, lung group, and bone group accordingly.Cell search system was used to detect CTC of all samples.The results were analyzed using sta-tistical methods.Results The CTC detection rate in blood samples was 35.5%(11/31) in liver group, 47.5%(19/40) in lung group and 68.8%(33/48) in bone group, respectively.The rate in bone group was significantly higher than those in the other two groups.The positive incidence of CTC detection in liver group was significant higher than those in lung and bone groups.Conclusion CTC detection is more suitable for patients of advanced breast cancer with bone metastasis be-cause of a higher sensitivity.A poor prognosis of liver metastasis patients could be predicted by CTC detection.
