目的:探讨侵袭性垂体腺瘤患者临床特征与其侵袭性关系,垂体腺瘤组织中基质金属蛋白酶-14(matrix metalloproteinases-14, MMP-14)、血管内皮生长因子(vascular endothelial growth factor, VEGF)表达水平与肿瘤侵袭性关系。方法选取2012年1月至2014年7月来我院治疗的96例垂体腺瘤患者为研究对象,50例非侵袭性垂体腺瘤,46例侵袭性垂体腺瘤,免疫组化法测定组织中MMP-14、 VEGF表达水平,分析其与肿瘤侵袭性的相关性。结果侵袭性垂体腺瘤组织中MMP-14、 VEGF强阳性表达率为28.26%、52.17%,总阳性表达率为65.22%、95.65%,显著高于非侵袭性垂体腺瘤组织中MMP-14、 VEGF强阳性表达率(14.00%、20.00%)及总阳性表达率(42.00%、56.00%)(P均<0.05); MMP-14、 VEGF为阳性情况下侵袭性垂体腺瘤体积、最大径显著高于非侵袭性组(P均<0.05), MMP-14、 VEGF为阴性情况下侵袭性垂体腺瘤与非侵袭性组,比较差异均无统计学意义(P>0.05);经相关性分析,侵袭性垂体腺瘤患者MMP-14与VEGF表达水平呈正相关(r=0.726, P<0.01)。结论侵袭性垂体腺瘤组织中MMP-14与VEGF表达增高, MMP-14与VEGF可作为辅助垂体腺瘤侵袭性判断生物学指标。

Objective To investigate the relationship between clinical features of invasive pitu-itary adenoma and its invasiveness, and between the expression levels of matrix metalloproteinase-14 ( MMP-14) and vascular endothelial growth factor ( VEGF) and the invasiveness of pituitary adeno-mas.Methods A total of 96 patients with pituitary adenoma who were treated between January 2012 and July 2014 in our hospital were examined.There were 46 cases of invasive pituitary adeno-ma and 50 cases of non-invasive pituitary adenoma.The expression levels of MMP-14 and VEGF were immunohistochemically detected, and the association of their expression levels with tumor inva-siveness was analyzed.Results In invasive pituitary adenomas, the strong positive rates of MMP-14 and VEGF were 28.26% and 52.17%, respectively, and the total positive rates were 65.22 %and 95.65 %, respectively, significantly higher than those in non-invasive pituitary adenomas in which the strong positive rates of MMP-14 and VEGF were 14.00% and 20.00%, respectively, and the total positive rates were 42.00% and 56.00%, respectively ( all P<0.05 ) .The maxi-mum diameter and the volume of invasive pituitary adenomas with positive expressions of MMP-14 and VEGF were significantly greater than those of non-invasive pituitary adenomas ( all P<0.05 ) .No significant difference was noted in the maximum diameter and the volume of pituitary adenomas between invasive pituitary adenomas with negative expressions of MMP-14 and VEGF and non-inva-sive pituitary adenomas ( P>0.05 ) .Correlation analysis revealed that the expression of MMP-14was positively associated with that of VEGF in invasive pituitary adenomas (r=0.734, P<0.01). Conclusion The expressions of MMP-14 and VEGF are increased in invasive pituitary adenomas and they can be used as auxiliary biological indicators for the invasiveness of pituitary adenomas.
