目的:研究尿和血胰岛素样生长因子-1(IGF -1)以及血清胱抑素 C(Cys C)水平变化在过敏性紫癜(HSP)和紫癜性肾炎(HSPN)中的意义。方法选取 HSP 患儿30例(HSP 组)、HSPN 患儿30例(HSPN 组)和健康儿童30例(对照组),分别采集尿和血,用化学发光法检测各组标本 IGF -1浓度;同时用全自动生化仪检测各组患儿血清Cys C、BUN、SCr 水平。结果①HSPN 组和 HSP 组尿和血 IGF -1的水平均高于对照组,差异均有统计学意义( P ﹤0.05)。②HSP 组和 HSPN 组血清 Cys C 水平均高于对照组,差异有统计学意义( P ﹤0.05)。③采用直线相关分析 HSPN组的血清 Cys C 水平与尿和血 IGF -1水平比较,存在正相关( r =0.759、r =0.906,均 P ﹤0.05),尿 IGF -1与血 IGF -1水平也存在正相关( r =0.661,P ﹤0.05)。结论 IGF -1为 HSPN 发病过程中促发因素,检测尿和血 IGF -1水平可作为过敏性紫癜早期肾损害的评价指标。

목적:연구뇨화혈이도소양생장인자-1(IGF -1)이급혈청광억소 C(Cys C)수평변화재과민성자전(HSP)화자전성신염(HSPN)중적의의。방법선취 HSP 환인30례(HSP 조)、HSPN 환인30례(HSPN 조)화건강인동30례(대조조),분별채집뇨화혈,용화학발광법검측각조표본 IGF -1농도;동시용전자동생화의검측각조환인혈청Cys C、BUN、SCr 수평。결과①HSPN 조화 HSP 조뇨화혈 IGF -1적수평균고우대조조,차이균유통계학의의( P ﹤0.05)。②HSP 조화 HSPN 조혈청 Cys C 수평균고우대조조,차이유통계학의의( P ﹤0.05)。③채용직선상관분석 HSPN조적혈청 Cys C 수평여뇨화혈 IGF -1수평비교,존재정상관( r =0.759、r =0.906,균 P ﹤0.05),뇨 IGF -1여혈 IGF -1수평야존재정상관( r =0.661,P ﹤0.05)。결론 IGF -1위 HSPN 발병과정중촉발인소,검측뇨화혈 IGF -1수평가작위과민성자전조기신손해적평개지표。
Objective To study the significance of urine and serum insulin - like growth factor 1(IGF - 1)and serum cystatin C(Cys C)levels in Henoch - Schonlein purpura(HSP)and Henoch - Schonlein purpura nephritis(HSPN). Methods Thirty HSP patients,25 HSPN patients and 30 healthy children(control group)were selected for the study. IGF - 1 concentration of specimens in each group was detected by chemiluminescence method;at the same time,the levels of serum BUN,SCr and Cys C were detected by automatic biochemistry analyzer. Results Urine and blood IGF - 1 levels in HSP group and HSPN group were significantly higher than the control group( P ﹤ 0. 05). Serum Cys C levels in HSP group and HSPN group were significantly higher than the control group( P ﹤ 0. 05). The linear correlation analysis showed that serum Cys C levels were positively correlated with urine and blood IGF - 1 levels in HSPN group( r = 0. 759 and r = 0. 906,respectively;all P ﹤ 0. 05);and the levels of urine IGF - 1 and blood IGF - 1 were positively correlated( r = 0. 661,P ﹤ 0. 05). Conclusion Urine and blood IGF - 1 can be used as the indicator for early renal damage in Henoch - Schoenlein purpura. The study suggests that IGF - 1 may promote the development of HSPN.
